You are enrolled to begin the Phase 2 Cardiac Rehabilitation Program soon. Prior to your first exercise session, you have been scheduled for an initial appointment to complete some paperwork, take some basic health measurements, and give you a chance to familiarize yourself with our program, location, and equipment.  This appointment will take approximately 1 hour.  Please bring:

• A list of all current medications
• Insurance Card
• If you have diabetes, please bring your glucometer
• Please wear comfortable clothing and walking shoes

Note:  Before your first appointment, we ask that you contact your insurance provider to obtain you policy’s coverage so you understand how much of the cost will be paid by your insurance and how much will be your responsibility.

Location:   1^st Floor-Heart Services

Directions: From the main parking structure, take the Skywalk on Level C into the hospital towards the Blue Elevators. Continue straight ahead and take a right at the end of the hallway. Follow the signs for Heart Services/Cardiac Rehabilitation. 

At the conclusion of this appointment, we will schedule you for your first exercise session.  If you are unable to keep this appointment, we kindly ask that you call a minimum of 24 hours in advance.
