Our team of highly qualified and caring professionals work as a team to promote our patients’ physical, emotional and spiritual well-being.
Physicians
We have three qualified physicians caring for all of our patients. Dr. Scott Clay, Dr. Abraham Cheong and Dr. Michael Raines.

Nurses
Nurse Case Managers guide the patient and their caregiver to create a customized plan of care that meets the patients’ goals and wishes. Our Registered Nurses conduct skilled clinical assessments, coordinate medications under the direction of the physician, arrange for medical supplies and equipment, and provide education and support for the entire family. Many of our hospice nurses are nationally certified as Hospice & Palliative Nurses and bring a high level of expertise to their work with patients and their families.
Certified Nursing Assistants
(CNA’s) provide routine, personal care for our patients wherever they live and visit as often as needed. These services may also include meal preparation and even light housekeeping duties.
Social Workers
Social Workers provide counseling and support for the patient, caregiver, and family throughout the course of the illness. At Hospice of the Golden Isles, we have several experienced full-time social workers, and our Chief Executive Officer is also a Licensed Clinical Social Worker (LCSW).
Hospice social workers are trained to help patients and families connect with a variety of community resources. They may also assist with completing advance directives and applying for healthcare and financial benefits.
Chaplains
Hospice chaplains provide non-denominational spiritual counseling and support for our patients and their loved ones. We warmly welcome your pastor, rabbi, priest or other clergy to participate in your care.
