Objective: We lack evidence that routine screening for depression in patients with coronary heart... more Objective: We lack evidence that routine screening for depression in patients with coronary heart disease (CHD) improves patient outcome. This lack has challenged the advisory issued by the American Heart Association (AHA) to routinely screen for depression in CHD patients. We assess the AHA advisory in the context of well-established criteria of screening for diseases. Methods: Using principles and criteria for screening developed by the World Health Organization and the United Kingdom National Screening Committee, we generated criteria pertinent to screening for depression in CHD patients. To find publications relevant to these criteria and clinical setting, we performed a broadly based literature search on "depression and CHD," supplemented by more focused literature searches. Results: Evidence for an association between depression and CHD is strong. Despite this, the AHA advisory has several limitations. It did not account for the complexity of the association between depression and CHD. It acknowledged there was no evidence that screening for depression leads to improved outcomes in cardiovascular populations but still recommended routine screening without providing an alternative evidence-based explanation. It ignored the paucity of literature about the safety and cost-effectiveness of routine screening for depression in CHD and failed to define the nature and extent of resources needed to implement such a program effectively. Conclusion: We conclude that the AHA advisory is premature. We must first demonstrate the efficacy, safety, and costeffectiveness of screening and define the resources necessary for its implementation and monitoring. Meanwhile, organizations representing cardiologists, psychiatrists, and general practitioners must coordinate efforts to manage depression and CHD through collaborative care, and work with the policy makers to develop the necessary infrastructure and services delivery system needed to optimize the outcome of depressed and at-risk-for-depression patients suffering from CHD.

They found that subjects with one or more metabolic abnormalities had a lower parasympathetic drive compared with those free of metabolic abnormality. The finding is not surprising. Vagus nerve mediated parasympathetic effect on the pancreatic -cells is essential in maintaining proper insulin secretion and glucose homeostasis. 2 Insulin causes release of hepatic insulin-sensitizing substance (HISS) that acts selectively on skeletal muscle to stimulate insulinmediated glucose uptake. This effect is mediated by the parasympathetic feeding signal to the liver. 3 Blockade of HISS release is said to produce HISS-dependent insulin resistance and may account for the majority of the insulin resistance seen in type 2 diabetes, hypertension, chronic liver disease, and obesity. 3 Depression has been associated with both increased insulin resistance and reduced heart rate variability (HRV), the latter being an indicator of low parasympathetic drive. Using data from the Cardiovascular Health Study, Carnethon et al 4 found that older adults who reported more depressive symptoms were more likely to develop type 2 diabetes than their counterparts who did not. This association was not fully explained by the established risk factors for diabetes or C-reactive protein. In a recent meta-analysis, Kemp et al 5 found that depression without cardiovascular disease was associated with reduced HRV, which decreases with increasing depression severity. Besides, low HRV after acute myocardial infarction may mediate the effect of depression on survival. 6 Considering that it is a phenomenon shared by interrelated conditions, namely insulin resistance, depression, and coronary heart disease, it may be fruitful to study diminished parasympathetic tone as a potential early biological marker explaining the association among these conditions.

Many psychiatric instruments were not tested for reliability and validity in subjects with comorbid medical illness, particularly coronary heart disease. They largely use scales of categoric or ordinal variables. Instruments used to assess coronary heart disease are considerably more objective and often use interval variables. By searching the websites of Circulation and the Journal of the American College of Cardiology, we entered the word "depression" on August 28, 2009. We ignored articles using "depression" in the context of cardiovascular concepts such as "ST-segment depression." By searching articles dating back to 1995, we selected publications that studied the prognostic association of depression and coronary heart disease. There were 5 relevant publications: 3 from Circulation and 2 from the Journal of the American College of Cardiology. The methods used to assess coronary heart disease (specifically, myocardial infarction) are largely homogenous across the studies, but the methods used to assess depression are heterogeneous. Parameters used to diagnose myocardial infarction and determine its severity are precise, objective, and reliable, whereas those used to assess depression and its severity exhibit less precision and lack comparable objectivity and reliability. This mismatch may compromise our understanding of the link between coronary heart disease and depression in depressed patients with comorbid coronary heart disease. We propose using precise instruments to identify and quantitate coronary heart disease as outcome variables to assess psychiatric interventions and to better define depression in depressed patients with comorbid coronary heart disease. This should lead to a better understanding of the link between depression and comorbid coronary heart disease.
Diffusion-weighted magnetic resonance imaging (MRI) has shown increased sensitivity in detecting ... more Diffusion-weighted magnetic resonance imaging (MRI) has shown increased sensitivity in detecting brain white matter disease compared to traditional T2-weighted MRI. Diffusion-weighted imaging (DWI) can quantitatively assess the microstructural integrity of white matter using the average apparent diffusion coefficient (ADC(av)), a measure of the extent to which water molecules move freely within tissue. On the basis of numerous studies suggesting white matter disease in bipolar patients, particularly patients with more severe illness, this study aimed to test the utility of DWI in assessing the white matter integrity of bipolar patients with severe illness. The existing MRI scans of eight bipolar patients and eight age-matched controls with neurological illness were examined retrospectively. ADC(av) values for pixels within white matter regions of interest (ROIs) were calculated and used to plot ADC(av) frequency histograms for each ROI. Mean ADC(av) values for the two groups were then compared by ANCOVA. The bipolar mean ADC(av) (0.855 +/- 0.051 x 10(-3) mm2/s) for combined white matter ROIs significantly exceeded that of controls (0.799 +/- 0.046 x 10(-3) mm2/s), while covarying for age (F = 4.47, df = 3, p = 0.025). This is the first report of an elevated ADC(av) in the white matter of a group of patients with bipolar disorder. In this group of patients with severe illness, increased white matter ADC(av) suggests microstructural changes consistent with decreased white matter integrity. DWI may be an additional, useful tool to assess white matter abnormalities in bipolar disorder.

Objective: We lack evidence that routine screening for depression in patients with coronary heart... more Objective: We lack evidence that routine screening for depression in patients with coronary heart disease (CHD) improves patient outcome. This lack has challenged the advisory issued by the American Heart Association (AHA) to routinely screen for depression in CHD patients. We assess the AHA advisory in the context of well-established criteria of screening for diseases. Methods: Using principles and criteria for screening developed by the World Health Organization and the United Kingdom National Screening Committee, we generated criteria pertinent to screening for depression in CHD patients. To find publications relevant to these criteria and clinical setting, we performed a broadly based literature search on "depression and CHD," supplemented by more focused literature searches. Results: Evidence for an association between depression and CHD is strong. Despite this, the AHA advisory has several limitations. It did not account for the complexity of the association between depression and CHD. It acknowledged there was no evidence that screening for depression leads to improved outcomes in cardiovascular populations but still recommended routine screening without providing an alternative evidence-based explanation. It ignored the paucity of literature about the safety and cost-effectiveness of routine screening for depression in CHD and failed to define the nature and extent of resources needed to implement such a program effectively. Conclusion: We conclude that the AHA advisory is premature. We must first demonstrate the efficacy, safety, and costeffectiveness of screening and define the resources necessary for its implementation and monitoring. Meanwhile, organizations representing cardiologists, psychiatrists, and general practitioners must coordinate efforts to manage depression and CHD through collaborative care, and work with the policy makers to develop the necessary infrastructure and services delivery system needed to optimize the outcome of depressed and at-risk-for-depression patients suffering from CHD.

They found that subjects with one or more metabolic abnormalities had a lower parasympathetic drive compared with those free of metabolic abnormality. The finding is not surprising. Vagus nerve mediated parasympathetic effect on the pancreatic -cells is essential in maintaining proper insulin secretion and glucose homeostasis. 2 Insulin causes release of hepatic insulin-sensitizing substance (HISS) that acts selectively on skeletal muscle to stimulate insulinmediated glucose uptake. This effect is mediated by the parasympathetic feeding signal to the liver. 3 Blockade of HISS release is said to produce HISS-dependent insulin resistance and may account for the majority of the insulin resistance seen in type 2 diabetes, hypertension, chronic liver disease, and obesity. 3 Depression has been associated with both increased insulin resistance and reduced heart rate variability (HRV), the latter being an indicator of low parasympathetic drive. Using data from the Cardiovascular Health Study, Carnethon et al 4 found that older adults who reported more depressive symptoms were more likely to develop type 2 diabetes than their counterparts who did not. This association was not fully explained by the established risk factors for diabetes or C-reactive protein. In a recent meta-analysis, Kemp et al 5 found that depression without cardiovascular disease was associated with reduced HRV, which decreases with increasing depression severity. Besides, low HRV after acute myocardial infarction may mediate the effect of depression on survival. 6 Considering that it is a phenomenon shared by interrelated conditions, namely insulin resistance, depression, and coronary heart disease, it may be fruitful to study diminished parasympathetic tone as a potential early biological marker explaining the association among these conditions.

Many psychiatric instruments were not tested for reliability and validity in subjects with comorbid medical illness, particularly coronary heart disease. They largely use scales of categoric or ordinal variables. Instruments used to assess coronary heart disease are considerably more objective and often use interval variables. By searching the websites of Circulation and the Journal of the American College of Cardiology, we entered the word "depression" on August 28, 2009. We ignored articles using "depression" in the context of cardiovascular concepts such as "ST-segment depression." By searching articles dating back to 1995, we selected publications that studied the prognostic association of depression and coronary heart disease. There were 5 relevant publications: 3 from Circulation and 2 from the Journal of the American College of Cardiology. The methods used to assess coronary heart disease (specifically, myocardial infarction) are largely homogenous across the studies, but the methods used to assess depression are heterogeneous. Parameters used to diagnose myocardial infarction and determine its severity are precise, objective, and reliable, whereas those used to assess depression and its severity exhibit less precision and lack comparable objectivity and reliability. This mismatch may compromise our understanding of the link between coronary heart disease and depression in depressed patients with comorbid coronary heart disease. We propose using precise instruments to identify and quantitate coronary heart disease as outcome variables to assess psychiatric interventions and to better define depression in depressed patients with comorbid coronary heart disease. This should lead to a better understanding of the link between depression and comorbid coronary heart disease.
Diffusion-weighted magnetic resonance imaging (MRI) has shown increased sensitivity in detecting ... more Diffusion-weighted magnetic resonance imaging (MRI) has shown increased sensitivity in detecting brain white matter disease compared to traditional T2-weighted MRI. Diffusion-weighted imaging (DWI) can quantitatively assess the microstructural integrity of white matter using the average apparent diffusion coefficient (ADC(av)), a measure of the extent to which water molecules move freely within tissue. On the basis of numerous studies suggesting white matter disease in bipolar patients, particularly patients with more severe illness, this study aimed to test the utility of DWI in assessing the white matter integrity of bipolar patients with severe illness. The existing MRI scans of eight bipolar patients and eight age-matched controls with neurological illness were examined retrospectively. ADC(av) values for pixels within white matter regions of interest (ROIs) were calculated and used to plot ADC(av) frequency histograms for each ROI. Mean ADC(av) values for the two groups were then compared by ANCOVA. The bipolar mean ADC(av) (0.855 +/- 0.051 x 10(-3) mm2/s) for combined white matter ROIs significantly exceeded that of controls (0.799 +/- 0.046 x 10(-3) mm2/s), while covarying for age (F = 4.47, df = 3, p = 0.025). This is the first report of an elevated ADC(av) in the white matter of a group of patients with bipolar disorder. In this group of patients with severe illness, increased white matter ADC(av) suggests microstructural changes consistent with decreased white matter integrity. DWI may be an additional, useful tool to assess white matter abnormalities in bipolar disorder.

The contribution of vascular risk factors to Alzheimer-vascular spectrum dementias is increasingl... more The contribution of vascular risk factors to Alzheimer-vascular spectrum dementias is increasingly being recognized. We provide an overview of recent literature on this subject. Overweight and obesity as well as underweight during midlife predict cognitive decline and dementia later in life. Hypertension during midlife is also associated with dementia later in life and the association is stronger for untreated hypertension. Calcium channel blockers, angiotensin converting enzyme inhibitors, and angiotensin-1 receptor-blockers may be particularly beneficial in diminishing the risk of dementia associated with hypertension. Studies have fairly consistently shown that type 2 diabetes is a risk factor for dementia. Episodes of hypoglycemia add to this risk. Regular physical exercise during any point in the lifespan protects against cognitive decline and dementia. Most benefit is realized with physical exercise during early and midlife. Dyslipidemia also increases the risk of dementia but the findings are less consistent. Findings on the possible benefit of lipid-lowering agents (statins) are conflicting. Earlier studies identified smoking as protective of dementia but recent better designed studies have consistently shown that smoking increases the risk of dementia. The association of vascular risk factors with dementia is more robust for vascular dementia than Alzheimer's disease. Heterogeneity of studies and lack of trials specifically designed to assess cognition as an endpoint make firm conclusions difficult. But considering the expected global burden of dementia and projected attributable risk of vascular risk factors to it, there is sufficient evidence to promote vascular risk factor reduction strategies as dementia prevention interventions.

Alhamdulillah, reading and trying to understand the Quran has been an area of my interest all my ... more Alhamdulillah, reading and trying to understand the Quran has been an area of my interest all my life. Over the years, my interest progressed from reading the translations of the Quran and browsing through a few Tafaseer selectively to trying to understand the Quran word-by-word and developing an intellectual sense of its main themes. During my pursuit that is ongoing I noted that the dictionaries of the Quranic root words either to brief or too elaborate. So, I decided to put together one myself one that was neither too simple nor too elaborate. It is a work in progress. So far, I have completed the root words of the first Arabic alphabet, Alif. InshaAllah, I plan to upload the PDFs of the root words by alphabet to the Archive.org website as I complete them. Once all the alphabets are done, I will create a complete dictionary of the root words and upload it as well. Further details about the dictionary provided in its introduction section.

This book has been written with a concern for the problem of irrational trends and practices in t... more This book has been written with a concern for the problem of irrational trends and practices in therapeutics. It focuses on the applied aspects of therapeutics, an understanding of which can facilitate physicians to approach and intervene in a clinical situation in a rational, scientific and logical manner.
'The book has been divided into three sections. The first section provides the fundamentals of rational therapeutics. In it are discussed how a patient should be approached and what factors should be taken care of to make a therapeutic decision as rational as possible. Effective communication i's an important factor for an effective therapeutic relationship; the section, therefore, starts with a discussion on the p{ocess of communication. The next few chapters discuss what factors are to be taken care of when planning a therapeutic intervention, how to remain up-to-date- about therapeutics, and how to write a prescription to make it maximally useful. Post-prescription-writing factors like observing the patient for desired and undesired effects of the treatment and monitoring him for compliance are overlooked by physicians in many instances and their significance in rational therapeutics is discussed in the last chapter of this section.
Section two of the book aims at providing physicians an insight into the factors that influence their therapeutic judgment and decision-making. Because -medical school training provides the knowledge and skills on the basis of which physicians practice therapeutics, the first chapter of the section emphasizes the significance of medical school training. In the absence of any formal ongoing training of physicians, and in view of the difficult access to unbiased and objective information on therapeutics, the pharmaceutical manufacturers bear a strong influence on physicians. The effects of this influence are discounted in the second chapter of the section. The last chapter of this section describes various patient and doctor factors that influence phvsicians' clinical approach. Governmental and regulatory factors also have a significant influence on the way therapeutics is practiced in a country, but because of the complexity of these factors and my limited awareness about them, they have not been discussed in the current edition of this book.
The last section of the book highlights the commonly observed irrational trends in the use of some common drugs. In addition to recognizing the irrational tend.s, guidelines on their rational use have also been provided. Drug categories that are discussed include antibiotics, psychotropics, nonsteroidal anti-inflammatory drugs, antiepileptics, and vitamins. Drugs used for the relief of gastrointestinal symptoms are also among the commonly misused group of drugs but, due to reasons, they could not be included in this edition of the book. The last chapter of this section focuses on the group of drugs that have been considered as "undesired" by experts and generally their use is not recommended.
