The supply chain network design and evaluation is a critical and difficult decision in a supply
chain management. In this paper, we present a new method for evaluating the performance
of a supply chain network. The main index is cost factors, which include four categories:
production costs, disruption costs, co-ordination costs, and vulnerability costs. In addition, in
