This paper presents a description and analysis of the different issues that must be
addressed in order to successfully simulate a surgical environment and to obtain reliable
measures of surgical skills. It then explores possible solutions to extend the current limits of
realism by using today's technology. The proposed techniques are demonstrated on a first
prototype system implementing the basic algorithms.
