HOME VISITS We ask patients to attend the medical centre unless housebound or too ill to go out. Remember that the doctors can see four or more patients at the medical centre in the time taken to do one visit. If you do need a visit, please phone before 10.00am to request this. Tell us if your condition is urgent. One of the doctors may phone you back to discuss your condition before visiting. ABUSIVE BEHAVIOUR Abusive behaviour to our Staff (Receptionist, Doctors and Nurses) will not be tolerated. Patients would not accept this behaviour from us and we therefore do not accept it from our Patients. Patients should be aware that all episodes of abusive behaviour will be documented on their record and may result in action being taken to deduct them from the Practice list. OUT OF HOURS On a Thursday afternoon and during Protected Learning Time our services are covered by Primecare on 0330 123 9219. If you require on-day treatment on a Thursday afternoon you may be asked to redial this number. For Out of Hours services please call NHS Direct on 111. We provide a wide range of services including general medical care, contraception, maternity services and minor surgery. We are also offering late evening surgeries. The building is accessible to patients using wheelchairs. There are disabled parking spaces near the main entrance. The GP reception has a low desk area and there are disabled toilet facilities in the building. Our Practice Nurses run a number of clinics to monitor and manage chronic illness (diabetes, heart disease, asthma, blood pressure, chronic chest disease). Also they provide health promotion, well person clinics, travel health advice, travel vaccinations, flu and pneumonia vaccinations, smoking cessation support and a wide variety of general nursing care. Our Health Care Assistants work under the supervision of a qualified nurse and carry out a range of procedures such as blood pressure checks, taking blood, urine testing, ECGs and breathing tests. We are a teaching practice and will often have attached students or GPs in training. Some consultations may be video recorded for training purposes. You will always be informed of this and will have the opportunity to decline if you wish. Our staff are duty bound to respect your privacy, dignity and confidentiality and to treat you with courtesy at all times. All patient information that we hold on paper or computer will be maintained securely and according to the regulations of the Data Protection Act. You have the right to know what information we hold on you. If you would like to see your records, please contact the Office Manager to arrange an appointment. HOW CAN YOU HELP US? Be on time for your appointment Please tell us if you cannot make your appointment Remember, one appointment is for one person Phone for a home visit before 10.00am Phone for test results after 1.30pm Use your repeat prescription slip for all repeat requests and allow at least 3 working days Tell us if you change your address or telephone number ROUTINE APPOINTMENTS When you contact the Practice you will also be asked for a brief description of your symptoms and how long you have had them, this information will enable us to book you with the appropriate clinician. We have Nurse Practitioners on duty every day working alongside our GPs they are able to manage the care of patients with minor illnesses, prescribing medication if necessary. We also have pre-bookable slots available up to 3 weeks in advance, which you will be offered if your condition is none urgent. We are unable to book patients to see a GP for a sick note or repeat prescription these should be ordered in advance by completing the slips provided on reception. EMERGENCY APPOINTMENTS All requests for urgent appointments will be triaged by the Duty Doctor on call. The Duty Doctor will telephone patients who need URGENT, same day help and if necessary will offer you an appointment later the same day. Please note URGENT Duty Doctor slots are not appropriate for sick notes, repeat medication or private letters. If the problem is life threatening, such as chest pain suggesting a heart attack, you think you may have had a stoke, you have severe breathing problems, you think your child has a meningitis rash, or someone has collapsed and is unconscious or not breathing, you should ring 999 straight away rather than call the doctor as prompt action can make all the difference. LATE ARRIVALS Appointments are booked at 10 min intervals, the Practice policy is that if you arrive more that 7 minutes late you will be asked to re-book as the doctor will not have enough time to see you. If your problem is urgent and needs to be dealt with that same day, please tell the receptionist who will book you onto the Duty Doctors list. Please be aware that you may have to wait to be seen. REPEAT PRESCRIPTIONS Repeat medication can be forwarded to the Pharmacist of your choice via an electronic system called EPS, please speak to your Pharmacist if you would like to sign up to EPS. You can also request repeat medication by completing the repeat slip attached to your prescription and bring it to reception to be processed. Alternatively if you have access to on-line services you can request repeat medication through the web site please note that acute medication cannot be ordered on line All prescriptions take 3 working days to process. Please remember: • URGENT PRESCRIPTION REQUESTS THE SAME DAY WILL ONLY BE DONE IN • • EXCEPTIONAL CIRCUMSTANCES. IF YOUR MEDICATION REVIEW IS DUE AND YOU HAVE NOT SEEN OR SPOKEN TO YOUR DOCTOR, YOUR NEXT PRESCRIPTION REQUEST MAY BE REFUSED. WE DO NOT TAKE PRESCRIPTION REQUESTS BY PHONE. REGISTERING AT THE PRACTICE If you live within the practice area and wish to register with us, please complete a registration form available from reception. The registration form and medical questionnaire both need to be fully completed and it will be very helpful if you can bring your medical card with you. There is a Blood Pressure, Height and Weight machine situated in reception for your use. Dependant on the information given, you may be asked to make an appointment to see the Practice Nurse.
