Constance Care Management is a professional geriatric care management service that provides highly responsive and personalized care tailored to meet the needs of seniors and their families. Constance Care Management is staffed by Connie Craig, LMSW, CMC, founder and owner and Trisha Fast, BA, CMC. Backed by over fifty years of combined experience we serve eastern Monroe County and the bordering areas of Ontario County.
