About this ebook
Designed to be a primer for mental health practitioners desiring to write treatment plans from a reality therapy perspective, Treatment Planning from a Reality Therapy Perspective, by author Michael H. Fulkerson, provides an explanation of how reality therapy treatment planning differs from traditional treatment planning models, which are usually based on the medical model and/or external control psychology.

Preface
With the second edition of the publication, I have changed the name from Treatment Planning from a Choice Theory Perspective to Treatment Planning from a Reality Therapy Perspective. I have two primary reasons for this title change. First, reality therapy is how it is listed in all the counseling theories textbooks. Calling reality therapy
by the name choice theory
is similar to calling behavior therapy
by the name stimulus-response theory.
None of the other methods of psychotherapy are referred to as just theories. In my opinion, it makes senses to refer to the practice of choice theory in psychotherapy as reality therapy. Second, reality therapy is a title synonymous with Dr. William Glasser and will be his legacy.
In addition to the title change, I have also made some other additions to the book. First, I have expanded chapter 2 by providing a more thorough description of using reality therapy in treatment planning, including a more extensive description of treatment outcome measures. Second, I have added two chapters. Chapter 5 will cover documentation of progress notes, and chapter 7 will compare reality therapy treatment planning with other models of psychotherapy. Finally, I have added page numbers and a table of contents to help make the book more organized and reader friendly.
Acknowledgments
My hope is that this book will be a helpful resource to those wishing to incorporate the method known as reality therapy into treatment planning. Although there have been many books written regarding a variety of topics based on choice theory/reality therapy, the information regarding the application of this approach on the topic of treatment planning has been sparse. My wish is that this book will help fill some of the void.
Thanks to Kim Olver, Bob Wubbolding, Brian Jones, Mary Kay Lamb, and Jim Evans for their suggestions, recommendations, and encouragement. This book is dedicated to the late Dr. William Glasser and Dr. J. Robert Cockrum.

Treatment Planning from a Reality Therapy Perspective
Synopsis
Treatment Planning from a Reality Therapy Perspective is designed to be a primer for mental health practitioners desiring to write treatment plans from a reality therapy perspective. This book provides an explanation of how reality therapy treatment planning differs from traditional treatment planning models, which are usually based on the medical model and/or external control psychology. Examples of how to write treatment goals and objectives that are precise and measurable are illustrated. Treatment plans from a reality therapy perspective have been field-tested by a clinician who has received numerous successful reviews from the Council for the Accreditation of Rehabilitative Facilities (CARF), the Office of the Inspector General (OIG), and several managed care organizations (MCOs).
I. Introduction
Most treatment planning books are based on the medical model and/or external control psychology approaches, such as behavior modification. Most psychotherapists use the diagnosis as the central guidance mechanism in the development of a treatment plan. Using the diagnosis of the client, the psychotherapist uses a treatment planner to identify goals and objectives associated with the mental illness diagnosis. The expectation is that the diagnosis will provide the clinician with the information to develop an appropriate treatment plan for the client.
Treatment strategies are based on what are often highly questionable diagnoses in which a treatment team of behavioral health professionals is unable to come to an agreement regarding the diagnosis. Furthermore, since these subjective diagnoses are based on treating mental illness, the goals and objectives are often not strength-based and may in fact have little to do with the client’s actual presenting problems.
My purpose for developing Treatment
