Report: We Must Improve Diagnosis in Health Care

The National Academy of Medicine is out with a landmark report on improving diagnosis in health care. The report concluded that most people will experience at least one diagnostic error in their lifetime, sometimes with devastating consequences. Diagnostic errors—inaccurate or delayed diagnoses—persist throughout all settings of care and continue to harm an unacceptable number of patients. For example: A conservative estimate found that 5 percent of U.S. adults who seek outpatient care each year experience a diagnostic error. Postmortem examination research spanning decades has shown that diagnostic errors contribute to approximately 10 percent of patient deaths. Medical record reviews suggest that diagnostic errors account for 6 to 17 percent of hospital adverse events.

I read your report on diagnoses and you are absolutely on point. But another, often overlooked, issue is, even when the doctor does make a correct and accurate diagnosis too much is lost in the translation. Either the patient or caregiver may not fully understand or be able to explain the information to another medical provider, especially while under stress in an emergency situation, assuming there is someone to even speak for the patient. Today's EMR portal makes accessing a patient's records easily accessible, assuming you know where to look. In ED situations you might even have to figure out whose information to seek out. Wouldn't it be great if everyone, or at least those with health issues, would carry their full medical file with them? Guess what, they can, and it can be done easily, conveniently and unobtrusively, and even more importantly, inexpensively. The CARE Medical History Bracelet provides just such an opportunity. The USB that is part of the bracelet has one section for general personal and contact information to help identify the patient and whom to contact in an emergency. The other section is designed to contain all the medical information, from past history through current medications, including test results, x-rays and MRI images.
