    LOAN MODIFICATION CHECKLIST

    Thank you for choosing Axia Financial, LLC to assist you with your loan modification.

    Detailed below you will find a checklist of items that will be necessary for us to obtain an
    accurate assessment of your current financial situation. It is extremely important to submit
    complete and accurate information, MISSING OR INACCURATE INFORMATION WILL
    DELAY THE PROCESSING OF YOUR REQUEST AND MAY AFFECT THE OUTCOME OF
    YOUR REQUEST.

    Please furnish this information to your Axia Financial, LLC representative as quickly as
    possible. Please allow time for the processing of your paperwork. Loan Modification is
    carefully achieved through correspondence with your lender.

    The following financial documents are required for both the borrower and co-borrower (if
    applicable).

    1.Mortgage statement or coupon (for each loan)


    2.Last 30 days of pay stubs for borrower/co-borrower/and-or/other household members
    3.Last two (2) year’s W-2’s for borrower and co-borrower
    4.Two (2) most recent federal income tax returns, including all schedules
    5.Tenant rent or other income proof, i.e. tenant lease agreement, (if applicable)
    6.Last three (3) months bank statements for checking and savings accounts of borrower
    and co-borrower
    7.Most recent retirement account statements
    8.Copies of current loan disclosures if any predatory lending is suspected
    9.Copy of current promissory note if on an adjustable rate mortgage (ARM)

    For the Self-Employed

    1.Federal Income Tax Returns for borrower and co-borrower


    • Copies of the original signed and dated tax returns, including all schedules for the
    two (2) most recent tax years.

    2.Profit and Loss Statement for the last three (3) months
    3.Last three (3) months’ business checking accounts of borrower and co-borrower (cover
    sheets only: showing the summation of deposits and debits)

    Paperwork That Will Be Completed With Affiliate Later

    1. Authorization to release information form completed for each loan


    2. Completed hardship letter (signed)
    3. Financial worksheet signed by borrower
    4. Blank financial worksheet, signed by borrower
