Element 1. Identified self and verified the childs identity. Performed hand hygiene. 2. Kept the dominant hand sterile and the nondominant hand clean for the procedure. Used only the dominant hand to manipulate the catheter. With the dominant hand removed catheter from the paper sheath, keeping it sterile. 3. Placed the distal end of the catheter in a cup of sterile saline to test the suction. 4. With the nondominant hand, removed the humidity source from the childs tracheostomy tube. 5. Hyperoxygenated the child before suctioning, using a resuscitation bag in the nondominant hand. Gave several breaths and removed the resuscitation bag. 6. Using the dominant hand, placed the suction catheter into the tube, making sure no suction was being applied at the time. Advanced the catheter no farther than 0.5 cm below the edge of the tracheostomy tube. 7. Once the catheter was in place, intermittently occluded the suction port and rotated the catheter to maximize contact between the tube and side holes of the catheter. 8. Removed the catheter and cleared the tubing with sterile saline. To prevent hypoxia, did not suction for more than 5 to 10 seconds. 9. Repeated as necessary, oxygenating between suctionings. 10. Alternatively, two people could do the procedure with one applying the resuscitation bag and the other performing suction. 11. Documented the procedure, the character of secretions cleared, how the child tolerated the procedure, and the childs vital signs.
