CASE PRESENTATION

Karin Troy

BACKGROUND INFORMATION
o85 year old Native American mother (widowed)
o5 children (3 girls and 2 boys)
oRetired from Public Schools of Robeson County
oHealth and abilities declining due to dementia progressing

PROBLEM STATEMENT
oClients dementia is progressing quickly and depriving the clients ability to perfrom
ADLs and IADLs.

oClient needs assistance with ambulation due to leg problems.


oClient also has several health problems that limit her ability to function.
oClient needs a ramp for ambulating in and out the home.
oClient needs assistance with floor repair in kitchen, bottom cabinets and bathroom
modifications.

SERVICES
oThe NC Baptist Men Red Springs, Mission Camp assisted client with building a ramp.
oThe client was approved for SA/IH to assist with sitter hours.
oSocial worker will make monthly visit to ensure that client is cared for properly.
oLumbee Tribe will assist client with floor repairs and kitchen cabinets.

STRENGTHS AND LIMITATIONS


oLives alone but a family member stays
overnight while others alternate
throughout the day.

oClient can not be left alone.


oUnable to ambulate alone.

oClient is alert to self and person most of oNeeds assistance with ADLs and IADLs.

oClient is disoriented due to dementia.
oClient rollator walker to assist with
ambulation.
oFamily assist with all ADLs and IADLs.

oOther medical diagnoses.

ASSESSMENT GOALS
oAssist client with ADLs and IADLs.
oMonitor changes and ensure that clients needs are met.
oProvide 24 hour supervision.
oMaintain accurate sitter logs.

oKeep receipts of payment for sitter fees.

BIASES
The social worker did not have any biases but as if the daughter thought she knew
more than the social worker.

VALUE DIFFERENCES
Client is very close to her family. The family seems to be very concerned about the
well being of their mother. Therefore, we both value a close knit family.

LIFE CHANGING EVENTS


oClient retired from Southeastern Regional Medical Center.
oClients closeness to family and work history has made her very polite and
understanding.
oClients leg condition caused her to lose ability to ambulate alone.
oClients dementia has drastically changed her life because she is not able to do things
she did in the past.

STAFF RECOMMENDATIONS
Client receive 24 hour care
Receive assistance with all ADLS and IADLs (cook, clean, finances, etc.)
Attend all medical appointments
Take medications as prescribed

Apply for PCS (Personal Care Services)

Social worker agreed with staff decisions. No suggestions were recommended.
