I, NESTOR A. JAVIER, of Legal Age, Married and Resident of Capitol
Site, Estancia, Kalibo, Aklan, after having been duly sworn to in accordance
with Law, hereby depose and say:
1. That I have been employed at Goncar Security Agency in ManocManoc, Boracay Island, Malay, Aklan from July 15, 2013 to June 30,
2014;
2. That as Security Guard under the Goncar Security Agency, I have
been assigned to various location in Boracay Island, Malay, Aklan;
3. That during my service as Security Guard, I am earning a Salary of
One Hundred Fifty Pesos (P 150.00) per Day equivalent to Eight (8)
Hours of Duty and Fifty Pesos
(P 50.00) Overtime Pay
equivalent to Four (4) Hours of Duty;
4. That after seperation from my employment, I havent received my
Fifteen (15) Days Salary from June 15-30, 2014 and 60 Days
Overtime Pay with Twelve (12) Hours of Duty per Day;
5. That I am executing this Affidavit to attest to the truth of the
foregoing facts to claim my unpaid Salary.
