    This certificate or appropriate parts as relevant must be completed and transmitted at each phase of the operation, and copied by fax for the attention of: Drilling Superintendent. Offshore Installation Manager.

    Note:

    Anchoring Operations include running, pulling and other work over well heads or flowlines performed by a MODU.
