INTRODUCTION

B.P. Koirala Institute of Health Sciences (BPKIHS) was established on January 18, 1993 and subsequently upgraded to an autonomous Health Sciences University on October 28, 1998 with a mandate to work towards developing socially responsible and competent health workforce, providing health care and engaging in health research. The Institute is located in Eastern Nepal and it has extended its continued health services through teaching district concept to Primary Health Care Centers, District Hospitals and Zonal Hospitals in six districts of the region. This University is aptly named after Bishweshwar Prasad Koirala, Nepal's visionary leader in social uplifting and a firm believer of national integration. This Institute is envisaged as one of the most successful examples of NepalIndia co-operation. The Health Ministers of the two countries signed the IndoNepal agreement for establishment of BPKIHS on the Hindu auspicious day of Mahashivaratri (March 10, 1994). BPKIHS has a vision of a self-governing, self-reliant international Health Sciences University aiming at attracting students and teachers from all over the world to its constantly innovative educational programs. BPKIHS has also been envisioned by the Nepalese Parliament as a centre of national importance to produce skilled manpower in health sector to meet the country's needs and also to function as a centre of excellence in the field of tropical and infectious diseases. BPKIHS has postgraduate, undergraduate, and university certificate programs. It has four colleges: Medical, Dental, Nursing, and Public Health. Its Central Teaching Hospital has 700 beds and well established major clinical and basic sciences departments. Postgraduate programs (MD/MS and M. Sc.) were started in 1999. All the MD/MS, M. Sc. and MPH degree are recognized by respective councils of Nepal and several MD/MS degrees (Anesthesiology, Dermatology and venerelogy, Orthopedics, Pediatrics, Surgery, Radiology, Psychiatry, Otorhinolaryngology, Medicine, Pharmacology and Gynaecology & Obstetrics) are recognized by Medical Council of India (MCI) as well. While selecting candidates for MD/MS postgraduate studies, the institute has given due recognition to those Nepalese medical graduates who have served the primary health centers and/or district hospitals under the government health system. MDS Program has been running since 2011 in Conservative Dentistry & Endodontics, Oral & Maxillofacial Surgery, Periodontology & Oral

Implantology, Pedodontics & Preventive Dentistry, Prosthodontics and Orthodontics. PhD in Basic Sciences and Tropical & Infectious Disease has been started from 2011. Launching of DM/MCh course is under process. The School of Public Health was established in 2005 and it is running a twoyear MPH program. The M. Sc. Nursing Program in Community Health Nursing, Psychiatry Nursing, Maternal Health Nursing, Child Health Nursing and Medical Surgical Nursing has already been started. The MBBS program of BPKIHS started in 1994, is recognized by Medical Council of Nepal, India and Sri-Lanka. It is listed in the World Directory of Medical Schools published by the WHO. The Institute started BDS program in 1999, which is recognized by the Nepal Medical Council. The duration of both BDS and MBBS Program is 4 years followed by a year of Compulsory Community Oriented Residential Rotating Internship, which is mandatory and to be completed successfully at BPKIHS and its Teaching District Hospitals. This feature is new and unique in this part of the world. B.Sc. Nursing and Certificate Nursing Programs were started in 1996 & 1999 respectively. These programs are well recognized and saleable within and outside the country. All the nursing degrees are recognized by Nepal Nursing Council. There are separate college complexes for both BDS and Nursing programs in the BPKIHS campus. Bachelor of Nursing (BN) program was started from 2012. The allied health sciences courses are B.Sc. MLT and B.Sc. MIT. In addition, BPKIHS organizes short-term academic programs/refresher courses for national and international students, doctors and faculty members. Continuing medical education on various aspects of health is a culture of BPKIHS. This University, aiming as a center of excellence in patient care, teaching, and research in health sector follows an integrated and holistic approach in academics and services. Situated in Dharan at the foothills having an area of about 700 acres, the University has a clean pollution free environment, and water and electricity supply round the clock. The institute is committed to its social responsibility and accountability. Candidates from disadvantaged groups are given opportunities for admission in most of the academic programs.

The Institute has innovative approaches of community-based training of students, and providing services to the local people through the concept of Teaching District Hospitals (learning by doing at community level). Accordingly, students of various programs visit and/or are posted to the health institutions under faculty/specialist supervision where on one hand they get opportunity to learn in the community set-up; and on the other hand, the local health institutions get regular qualified health workforce to provide the quality services by a BPKIHS team of faculty/specialist and interns.

VISION, MISSION AND GOALS OF BPKIHS

Vision BPKIHS has been visualized as A self-governing, self-reliant International Health Sciences University attracting students and teachers from all over the world to its constantly innovative educational programs. A national center for providing quality health services both, tertiary as well as primary, and developing replicable and sustainable models of integrated health systems sensitive to the needs of both the individuals and the community living in urban as well as rural areas. A prestigious center for biomedical and health system research, and as a center of excellence for tropical and infectious diseases. A complete health development project through its teaching districts, which will catalyze an environment-friendly human development initiative resulting in enhancement in the quality of life of the people of Nepal and, those working in BPKIHS to ensure a high level of commitment to excellence and dedication to service. Mission The main mission of BPKIHS is to improve the health status of the people of Nepal by providing holistic health care through training of compassionate, caring, communicative and socially accountable health workforce acting as agents of change and through advancement in research and innovation in service as well as education to ensure healthy individuals and families by collaborating with all stakeholders.

Goals Educational Goals In terms of achieving the educational goals of the Institute, both the present curricula and future educational programmes are to be need-based, integrated, community-oriented and partially problem-solving in line with innovative medical education programmes epitomized in the Edinburgh Declaration of 1988. These are the basic tenets on which BPKIHS curricula are based. In the area of service delivery, this Institute is committed to provide services not only to those coming to its teaching hospital but also to those who are not able to do so. The Institute does this by reaching out to the community, that cannot reach the Institute. It is not only reaching out to the sick in the community but is committed to the active promotion of positive health and through it, improvement in the status of the community health that will serve as a measure of attainment of our service goals. The faculty at BPKIHS is expected to monitor health indicators such as Infant mortality rate, crude birth rate, crude death rate, blindness, deafness and disability rates etc. on an ongoing basis much in the same way as we monitor our individual patients in terms of established clinical parameters. Research goals In the field of research, high priority is accorded to the areas requiring greater attention to solve the existing and emerging health problems threatening the well being of large population groups using multi-disciplinary, integrated and problemsolving approach; greater emphasis is placed on research which will lead to application of available knowledge for the benefit of large number of people.

CURRICULUM

In accordance with the national vision & goal of education and the Institution's philosophy, BPKIHS Certificate in Nursing curriculum is intended to develop a self reliant & committed nurse and as well as a responsible citizen and a contributing member of the society. Through a systematic process of study and experience, knowledge is gained, skills are developed and attitudes are inculcated by providing innovative learning environments. The curriculum reflects the basic principles and the elements of Primary Health Care (PHC) as well as the level of prevention & health care in a comprehensive health care system. This course will provide the basis for advanced education in nursing practice, teaching, research and administration.

Note: i) A candidate cannot choose more than one option among 1(1.2 to 1.4) under Heavily Subsidized Tuition Fee Scheme. ii) Candidates applying for 1 (1.2 to 1.3) under HSTFS are not eligible to apply FTFS category.

ELIGIBILITY CRITERIA

Female candidates who have passed SLC examination with a minimum of 45% marks in aggregate and with Science and Mathematics as principal subjects in class 10 are eligible to apply for Certificate in Nursing Program. Eligibility for reserved seats under HSTFS: a. Hill District Category : Candidates from the following selected hill districts and passed class 8, 9 and 10 from public schools situated in these districts are eligible for hill district category seats. Selected Hill Districts are: Bajhang, Bajura, Darchula, Dolpa, Humla, Jajarkot, Jumla, Kalikot, Manang, Mugu, Mustang, Rukum, Solukhumbu. b. Criteria for Dalit Group: Candidates from Dalit Group as listed in "Hill Tribes and Terai Tribes" by the National Dalit's Commission, Government of Nepal are eligible for the seats allotted to this group. c. Eligibility Criteria for the BPKIHS Faculty/Staff Seat: One seat in Certificate Nursing Programme will be filled in by a daughter of BPKIHS Nepali Faculty/Staff who meets eligibility criteria of open category and meets other criteria for BPKIHS Faculty/Staff Seat, on merit basis.

One seat is reserved in all the undergraduate educational programmes at BPKIHS for son & daughter of BPKIHS Nepali Faculty/Staff who have continuously served the Institute for 3 years and are presently serving the institute. The selection of the candidate in this seat will also be completely on the basis of merit of the Entrance Examination. After the selection of the child the staff has to continue his/her services at least for further 4 years if the Institute requires his/her services. In case of failure to comply to this rule, the candidate from faculty/staff seat will have to pay tuition fee as decided by the Institute. Note: Dalit/Hill District category candidates- A copy of certificate issued by the Dalit Commission (as applicable) as well as a verification certificate from the district administration authority of concerned district must be attached with the application. A certified letter from concerned district education administration is a must, to prove the candidate has studied classes 8, 9 & 10 in the Public School in the specified district for candidates applying for Hill District category.

HOW TO APPLY

1. Candidate can submit her application online only. Please read the prospectus carefully before proceeding to apply. Procedure for Online Application Instructions to submit online application is available on webpage www.bpkihs.edu. Follow the link provided on home page to proceed. Please follow the instructions carefully before filling the online application forms. Scanned copies of all the essential certificates/documents (as mentioned in Page. No. 8), photo & signature must be attached along with the application as and where necessary. The full signature of the selected candidates will be verified at the time of entrance examination and admission. A photograph of the candidate will be taken during the entrance examination for verification of her identity and for keeping institutional record. 2. Submission period for application is from 2nd June 2013 to 5th July 2013. However, forms can be submitted with a late processing fee of Rs. 1,000/- (US$15 for FTFS or equivalent Nepali/Indian currency) extra from the 6th July 2013 till 19th July (Friday) 2013. No application will be received after 19th July 2013. Once the form has been submitted, it will be scrutinized and the status of the application will be intimated within 7 days. Candidates must bring and deposit the original bank voucher/SWIFT transfer receipt at the time of Entrance Examination. Admit card will be electronically issued/can be downloaded after 28th July 2013 following verification of the uploaded certificates/bank voucher/SWIFT transfer and the application form. Important: In case the last dates mentioned-above happened to be a public holiday, the next working day will be considered the last date unless otherwise mentioned.

3. The mode of payment for entrance examination fees is as follows: i. The fee should be deposited in favor of B. P. Koirala Institute of Health Sciences, Dharan by Bank Voucher. OR A SWIFT transfer should be made in favor of B. P. Koirala Institute of Health Sciences, Dharan" in any one of the banks mentioned below. ii. The Name and Registration ID of the candidate should be clearly mentioned in the Bank Voucher or SWIFT transfer document. iii. 4. Please go through the prospectus and application form carefully before completing the forms. 5. Write date of birth only according to the English Calendar (A.D.). 6. Write your choice of tuition fee scheme of the program in which you want to apply in the box given in the application form. The scheme cannot be changed after the last date of form submission. The application form, duly filled in, should be accompanied by the following documents: 1. A Scanned copy of certificate issued by school showing the date of birth of the candidate. 2. A Scanned copy of the mark sheet of SLC or equivalent (Internet printout is not acceptable). 3. A scanned copy of citizenship-certificate or Passport. 4. Foreign nationals must submit a photocopy of citizenship or passport or proof of residential document with photograph along with the application form. 5. Dalit category candidates- A copy of certificate issued by the Dalit Commission as well as a verification certificate from the district administration authority of concerned district must be attached with the application. 6. Hill District category candidates- A certified letter from concerned district education administration is a must, to prove the candidate has studied classes 8, 9 & 10 in the Public School of the specified hill district. 7. Instructions for Photographs a. One recent color passport size photograph, taken after 1st June 2013, with light background is required. b.Black & white / Polaroid photographs are NOT acceptable. c. The photograph must be snapped holding name of candidate and date of photograph taken on an A4 size placard. In case name and date are printed on the photograph after taking it, the application will be rejected. d.The name and date on the photograph should be legible.

NOTE: Candidate must upload photograph, signature and other required documents. Do not make any mistake while uploading these documents. The scanned copy of photograph, signature and other required documents should be in JPEG or JPG file format. Candidate must have softcopy/digital of passport size photograph and save it as "Photograph Name" with the maximum size limit being 500 KB. a. Photograph must show the face and both ears clearly. b. Photograph should not have cap, goggles or anything that does not show the face clearly. c. Originality of the photograph should not be altered. d. Any application received with photograph submitted other then above will be rejected. Instructions for Signature Put candidate signature with a black ball point pen on blank white paper in an area of 2"x1". Scan that paper, crop signature area of 2"x1" and save it as "Signature Name". Keep size of signature in between300-400 KB. Important: 1. In case any form/card required to be uploaded along with the application form is not enclosed with the application form, the application will be summarily rejected. 2. If any candidate is found to have furnished false information or certificate or found to have withheld or concealed some information in her application form, she shall be debarred from getting admission. 3. No recommendations are required except the above mentioned documents which should be sent with the application form. Any attempt

on the part of candidate to influence the process of admission directly or indirectly will be treated as a disqualification. All entries and signatures of the applicants, guardian and attesting officer along with office stamp of the attesting officer should be made in proper places. Write date of birth only according to the English Calendar. Incomplete application forms will not be accepted. A candidate can contact Academic Administration Division through phone or email in case of any problem in submitting form. In all disputes pertaining to this Entrance Examination the decision of the Entrance Examination Committee of B.P. Koirala Institute of Health Sciences shall be final. Admission of the candidates to the Entrance Examination is provisional subject to their being found otherwise eligible for admission to the course concerned. No electronic devises e.g. mobile phones, calculator etc. will be allowed in the examination hall. Ball Pen for writing answer sheet will be provided at the exam hall. Candidate should not bring Pencil, Ball Pen, Pen, Eraser, Sharpener etc.

ENTRANCE EXAMINATION

a. Structure of the Examination


The Entrance Examination comprises of multiple choice questions in General Science, Mathematics, English and Knowledge about Health. Centers for Entrance Examination will be at Dharan. The exam centre will be indicated in the Admit Card. The Entrance Examination will be of 1 hour duration and will have one paper consisting of 100 objective type questions. The question paper will be in English. The distribution of marks will be as follows: Subject General Science Mathematics English and Knowledge about Health Total Marks 40 20 40 100

The general standard of the Entrance Examination will be that of the SLC Examination. No syllabus for the examination has been specifically prescribed by BPKIHS.

SELECTION PROCESS

a. Steps in selection Process: A master merit list of all candidates who appeared in the entrance examination and scored at least 40% marks (cut-off mark) will be drawn. 1. In the first step, candidates according to the number of seats available in open category under HSTFS will be selected for admissions in order of merit irrespective of their ticking in different options under this scheme. 2. Candidates in each reserved-category option under HSTFS will be selected for admission in order of merit. 3. The candidates who are not selected in open and reserved category seats but have also opted for a FTFS seat will be considered for drawing the FTFS merit list, and accordingly admissions will be given. The seat allotted to each category in 1.2, 1.3 and 1.4 if not fulfilled due to unavailability of candidates those seats will be added to the open category 1 (1.1). In case any applications are not received in FTFS category till the last date of application, those seats will be transferred to HSTFS (Open Category). Similarly, any vacancy created in the end due to non-admission by any

candidates in the waiting list of FTFS category the seats will also be transferred to HSTFS (Open Category). Admission will be confirmed only after paying the required amount of fees by the stipulated time and date. b. IN CASE OF TIE If there is a tie between more than one candidate for the last place for admission by virtue of their having obtained equal marks in the Entrance Examination , the candidate who has secured the higher marks in the General Science Section of the Entrance Examination will be considered to have qualified for admission. In the event of second tie, the marks obtained in Mathematics section of the Entrance Examination will be considered. In the event of third tie the marks obtained in the English section will be considered. Similarly, in the event of fourth tie, SLC (or equivalent examination) percentage will be considered, and higher percentage will be in the higher rank.

PROCEDURE FOR ADMISSION

The result and notice for admission will be made available at the following places by the same day: 1. Notice board of the Academic Division, BPKIHS, Dharan 2. Website of the Institute (www.bpkihs.edu) The successful candidates are required to report to the Office of the Dean, Academic Division, BPKIHS, Dharan with all their original academic certificates and citizenship certificate within the stipulated date and time for completing admission formalities, viz. verification of original certificates, medical check-up and payment of fees. Please note that it will take about three days to get complete report of the Medical Check-up, and, therefore try to contact the Office of the Dean, Academic Division, BPKIHS, Dharan as early as possible. If any candidate selected for admission fails to join the course for any reason by the last date, the top candidate on the waiting list will fill her place. At the time of admission original documents of Schooling and SLC (certificate and mark-sheet) and citizenship must be deposited to the Institute and shall be returned after university registration & as per all other regulations. Candidates desiring to leave the course any time after completion of the admission process will have to pay penalty as per regulations.

10. MEDICAL EXAMINATION


All the selected candidates shall have to undergo medical examination by a Medical Board set up by the Institute. Criteria for fitness are as per the protocol set up by the Board. In case of further investigation, the Board may refer the subject to other institutions. In such case, all the expenses must be borne by the candidates. If, in the assessment by the Medical Board, a candidate is found medically unfit to be admitted to the course, she will not be admitted to the course. In that case, in her place candidate on the waiting list will be admitted on merit basis. Please note that it will take a minimum of three days to complete medical examination. The selected students must take all vaccinations as advised by the Medical Board.

11. FEES AND EXPENSES


Each candidate selected for admission shall have to pay the following fees and dues immediately after the announcement of list of selected candidates and after they are declared medically fit by the Medical Board, failing which, her admission shall be canceled and the candidate on the waiting list shall be admitted in her place. 3. Tuition fee & Hostel fee can be paid in two equal installments, the first being paid at the time of admission and at intervals not exceeding six months thereafter. All other fees should be paid at the time of admission. The last date of payment of fees must be strictly adhered to. No extension is possible unless an exceptional case by prior approval from the Registrar not extending a period of 15 days from the last date with payment of fine Rs. 50/- per day for the 2nd installment. For further delay after 15 days the following fine shall be imposed: For HSTFS For FTFS Rs. 100/- per day Rs. 500/- per day

4. Students not paying fees along with the mess charges on time will not be allowed to sit in the annual examination. Admit card for such students will not be issued. 5. Students of all schemes who have to repeat the academic year due to failure or any disciplinary action should pay fees as follows: a. Tuition Fee: 15% of the total tuition fee b. All other fees: 100% as mentioned in the prospectus

12. HEALTH INSURANCE


All candidates selected for admission in BPKIHS will have to get their health insured comprehensively before being admitted in to the course and they will have to renew their health insurance policy yearly during their whole study period in BPKIHS. The students must make sure that they comply with insurance policy of their choice.

13. DURATION OF COURSE


The course of studies leading to Certificate in Nursing degree of the B.P. Koirala Institute of Health Sciences shall last for a minimum of 3 academic years. The maximum limit of the study period of this course will not be more than 6 years from the date the course started. A student declared unsuccessful at the first year examination will be allowed up to a maximum of three more attempts. Any student who fails to clear the first year examination in up to four attempts (including the first one) will be required to leave the course.

14. GENERAL GUIDELINES FOR ANNUAL EXAMINATION


1. Attendance: In order to be eligible to appear in the annual examination the student should have at least 80% attendance in each of the three categories of classes: theory, practical and clinical posting & field postings separately in individual subject during their academic year. 2. Students must secure 50% marks in theory & practical examinations in each subject/paper separately to pass the final assessment. 3. A supplementary examination will be held within 6 weeks from the date of publication of result of examination for 1st, 2nd, 3rd year. 4. No student will be allowed to appear in the supplementary examination if she has not appeared in the regular annual examination except where there is exceptional condition either due to illness of the candidate or bereavement in the family or any other such compelling circumstance. The final decision pertaining to this matter will rest on the Rector's office. 5. Any student who fails in the supplementary examination will have to reappear in the subject(s)/paper(s) in which she has failed in the next final assessment & all the rules governing the attendance, internal assessment and payment of fees are as per rules. 6. A student declared unsuccessful at the first year examination will be allowed a maximum of three more attempts. Any student who fails to clear the first year examination in up to four attempts (including the first one) will be required to leave the course. The registration of those candidates failing to complete the course within 6 years from the academic year of admission will be cancelled. 7. The contribution of internal assessment & annual examination to the final assessment will be 30% & 70% respectively. Separate re-internal assessment for failures in the final assessment will not be taken, and in their case the 30% marks to be considered for internal assessment will be derived from the marks secured in the concerned subject/paper in the regular annual examination. 8. Certificate of merit will be awarded to those students scoring a minimum aggregate of 75% on total marks in the annual examinations in the first attempt & also being on the 90th percentile & above.

15. HOSTEL
There are separate hostels for boys and girls. Staying in the hostel is compulsory for all the students admitted to BPKIHS in order to inculcate habits of scholarship and a sense of belonging. The students themselves manage hostel-mess. The Institute has provided a well-furnished kitchen. Students are advised to use

electricity and water rationally to save energy. The students admitted must strictly follow a code of conduct as per BPKIHS rules & regulation. Entire campus is ragging free zone. Ragging in any form is punishable. Anybody found doing such activities will be taken seriously. Possessing or riding motorbike/scooter inside the campus by the undergraduate students is strictly prohibited. Any violation may be liable to punishment as per the institute rules.

16. LIBRARY
The Central Library is well-equipped and maintained with good collection of medical books and other resources. It is providing services for 15 hours a day from 8:00 am to 11:00 pm (Sunday Thursday). On Fridays, it opens from 8:00 am to 1:00 pm and on Saturdays, from 8:00 am to 6:00 pm except on public holidays. More than 70 titles of International and Indian medical journals are being subscribed to the Central Library annually. Apart from these resources, approximately 8000 biomedical health sciences free full text journals, books and medical databases are accessible from WHO/HINARI. The Electronic Information Services (EIS) has been upgraded in library installing wi-fi devices and also facilitated to have access to HINARI Resources and the Internet from residence, hostel and departments through wi-fi and LAN connections. Each department has its own Departmental Library with essential collection of books and journals, which is updated regularly. In addition to this, the Library is conducting various activities to enhance the knowledge and skills of users for literature search, electronic information access and so on.

17. VISA FOR FOREIGN STUDENTS


Foreign students have to pay necessary visa fee during their study period. Institute will help the students with necessary communications to the concerned authority.

19. ENTRANCE EXAMINATION COMMITTEE


The Entrance Examination Committee of the University governs selection of candidates for admission to all academic programs. The Committee invites and receives applications, screens eligibility, rejects applications not meeting the eligibility criteria, fixes examination date, time and its centers, conducts entrance examinations, publishes results, and recommends admission of successful candidates. The Committee also reserves the right of debarring candidate from entrance exam and cancellation of admission of any candidate at any time, if, after inquiry it is proven that the information provided by the applicant is false. The decision of the Entrance Examination Committee shall be final and binding. Note: - During the examination/before/or later, if a candidate is found indulging in any unfair means, she shall be deemed to have used unfair practice at the examination. Such candidates would be expelled and debarred from taking the examination in future too and shall also be liable for criminal action and/or any other action deemed fit by the institute. We have no agents or middlemen for admissions.

20. DHARAN
Dharan is a lovely town situated at the foothill of Siwalik/Chure Mountains in the Eastern Region of Nepal. This town is one of the most important industrial, economic and educational centers in the region and a gateway to the eastern hills & mountains of Nepal. Dharan has climate and vegetation a mixture of tropical and sub-tropical. It has very pleasant weather throughout the year. The summer is warm having a maximum average temperature 32oC and the winter very pleasant having a minimum average temperature 10oC. The population of Dharan Municipality is more than 125, 000 having mixed ethnicity and religion. The cultural diversity is another important aspect of Dharan. Dharan is connected with a domestic airport in Biratnagar few km north from Indian state of Bihar, about 50 kilometers south (1 hour drive) from Dharan. The same road continues to north connecting Bhedetar (a hill station hour drive from Dharan), Dhankuta (about 52 kilometers further north, Hile and Basantapur. Dharan is easily approachable from Siliguri, New Jalpaigurhi in West Bengal and Katihar in Bihar. Trip to Dhankuta, Hile and Basantapur would definitely be a remarkable one for anyone to see picturesque mountains of the Himalayan ranges. The panoramic views of the majestic mountains and the Himalayans seen from Hile & Basantapur will never be enough to anyone who loves nature. Indeed this is a heritage of Hindu and Buddhist pilgrims; there are Mosques and Churches as well. Barahakshetra, Dantakali, Pindeshwar, Budha Subba and Panchakannya temples are the famous Hindu holy places one should not miss to visit.
