This notice is to inform you of a serious problem in my apartment. Since, ________________ I have been without and/or lacking sufficient [state here exact problem and the part of apartment affected by problem.] I would like you to correct this problem immediately so that I might enjoy the full use of my apartment These problems make my apartment not reasonably fit and habitable and are violations of the Chicago Residential Landlord-Tenant Ordinance, Ch. 5-12-100 (f) and my lease agreement. Therefore, I am requesting that you remedy this problem within 72 hours. Otherwise, under the law, my lease will terminate at the end of that 72 hours period. I will then vacate the premises within 30 days thereafter. Please keep in mind that according to the CRLTO, Ch 5-12-150, A landlord is prohibited from retaliating by terminating or threatening to terminate a tenancy, increasing rent, decreasing services, bringing or threatening to bring an eviction action, or refusing to renew a lease agreement. Please inform me as to how you intend to correct this problem.
