Instructions: This evaluation form is to be accomplished by the immediate supervisor of the trainee and to be
returned to the Industry Immersion chairman. Circle the rating that describes the trainee most accurately. Get
the sum of the value for all responses and record in the total score section. To get the grade equivalent refer to
the Table on Numerical Rating on the next sheet.
