1. GENERAL

This requisition covers the minimum requirements of B2 Pavement Replacement Work to be


performed and completed by SUBCONTRACTOR.
SUBCONTRACTOR shall perform and complete the work strictly in accordance with the
requirements contained in this Requisition.

2. SCOPE OF WORK AND SUPPLY

2.1. SUBCONTRACTORs Scope

2.1.1 Direct Work

Outline of Scope of Direct Work is as follows, but not limited to:


a) Demolish existing paving and exavation work
b) Dispose unused concrete and soil to out side PTS area
c) Excavation and compation work
d) Concrete work
e) Install steel plate and rubber (material supplied by SAE)
f) Supply and install all material that mention in work volume (see attachment -2)

The SUBCONTRACTOR shall prepare necessary procedure for execution work.


Subcontractor shall provide surveyor for construction activity.

2.1.2 Indirect Work

SUBCONTRACTOR shall provide some indirect work required to perform the Work at his
own cost and expenses.

2.2. SAEs Scope

a) Design and Engineering


Basic engineering including preparation of specifications and lay-out drawings shall be
excluded from SUBCONTRACTOR's scope of work and shall be performed by
CONTRACTOR and furnished to the SUBCONTRACTOR.

b) Drawing
Outline drawing of the work are listed in standard and typical drawings and as reference
only. CONTRACTOR will issue drawings for construction progressively in accordance
with SUBCONTRACTOR's scope of work after the contract has been awarded.

3. MANPOWER

3.1. SUBCONTRACTOR shall provide all of direct manpower including unskilled labor, skilled labor
up to foremen that are needed to perform and complete the Work.

3.2. The manpower mobilized by SUBCONTRACTOR shall have sufficient skill in accordance with
his specialization or job description. If in the opinion of CONTRACTOR, any manpower is

deemed unacceptable or has no skill in accordance with his job description,
SUBCONTRACTOR shall replace him with acceptable and capable manpower immediately.

3.3. SUBCONTRACTOR shall have dedicated superintendent and if necessary, they shall assist to
CONTRACTORs superintendent for clarifications or meetings with client.

3.4. SUBCONTRACTOR shall take a training program to maximize his manpower capability and
efficiency.

3.5. SUBCONTRACTOR shall provide all of safety accessories, necessities, insurance and others for
his manpower.

3.6. SUBCONTRACTOR shall complete his manpower with necessary qualification and with
certificates related to his job description and in accordance with the law or regulation.

3.7. If it is necessary and/or at CONTRACTORs request, the SUBCONTRACTOR shall mobilize


additional manpower to achieve work progress at his own cost and expenses. If the
SUBCONTRACTOR cannot mobilize additional manpower, CONTRACTOR has the right to
mobilize the manpower and the cost related to the manpower mobilization by CONTRACTOR
shall be back charged to the SUBCONTRACTOR.

4. HEALTH, SAFETY, ENVIRONMENT AND SECURITY (HSSE) PROGRAM

CONTRACTOR gives safety, health and environment on the Jobsite the highest priority, regardless
of the location and nature of the Jobsite and its environment. Our constant objective is zero
accidents, injuries and fatalities, and an effective environmental protection program. The
SUBCONTRACTOR will be required to demonstrate a comprehensive awareness of safety, health,
and environmental protection objectives by means of a comprehensive and effective program
complying with the CONTRACTOR HSE procedure. SUBCONTRACTOR shall propose HSE Plan and
HSE Program that will be applied in the performance of the Work for Contractors review and
approval according to CONTRACTORs HSE Project Plan. All of the SUBCONTRACTORs staff and
labor shall aware of the HSE Program. SUBCONTRACTOR shall provide equipment, tools and
apparatus needed to support the HSE Program.

SUBCONTRACTOR shall pay attention to the security of his properties (material, construction
equipment, tools, etc.). SUBCONTRACTOR shall assign his staff to in charge fully in security and
make coordination with CONTRACTORs security. Any loss of the SUBCONTRACTORs properties is
the SUBCONTRACTORs responsibility.

SUBCONTRACTOR shall protect his manpower, material including material supply by


CONTRACTOR, construction equipment, tools, and other properties from weather, damage,
accident, loss, thievery, etc. by providing necessary protection.
All cost and expenses related to the HSE and security program shall be the SUBCONTRACTORs
responsibility

5. CONSTRUCTION EQUIPMENT

5.1. SUBCONTRACTOR shall provide all construction equipment those are needed for execution
and completion of the Work. SUBCONTRACTOR shall propose the construction equipment for
CONTRACTOR review and approval.

5.2. SUBCONTRACTOR shall provide the construction equipment including its accessories,
operator, consumable, spare parts, maintenance, permits and others related to the operation
of the construction equipment at his own cost and expenses.

5.3. All construction equipment items as proposed by SUBCONTRACTOR are to be inspected and
maintained in accordance with applicable codes and regulations. Records of all inspections
and maintenance should be provided and maintained for CONTRACTOR review and approval.

5.4. SUBCONTRACTOR shall include a procedure related to the construction equipment


inspection, operation and maintenance in his submitted construction safety program. It shall
be consistent with the manufacturers' recommendation and CONTRACTOR requirements and
be incorporated after SUBCONTRACTOR's proposal reviewed by CONTRACTOR.

5.5. Operator of all cranes and other heavy equipment shall be qualified to operate the assigned
equipment in accordance with government regulation. Crane and operator shall have valid
certification from DEPNAKER. SUBCONTRACTOR shall provide all licenses required for
operator at his own cost and expenses.

5.6. The construction equipment to be used will be subject to inspection before work starts under
the contract. If in the opinion of CONTRACTOR, any of equipment is found unsuitable and it
would interfere with desire results, the SUBCONTRACTOR shall replace them with suitable
equipment immediately.

5.7. If it is necessary and/or at CONTRACTORs request, SUBCONTRACTOR shall mobilize


additional construction equipment to achieve work progress at his own cost and expenses. If
the SUBCONTRACTOR cannot mobilize additional construction equipment, CONTRACTOR has
the right to mobilize the construction equipment and the cost related to the construction
equipment mobilization by CONTRACTOR shall be back charged to the SUBCONTRACTOR.

6. TEMPORARY FACILITY
6.1 SUBCONTRACTOR shall provide temporary work at his cost and expenses for items below
but not limited to :
- Temporary office.
- Temporary shop and storage.
- Temporary toilet
- Smoking shelter.
- Any other temporary facilities needed by Subcontractor

6.2 To prepare and provide the temporary work, the SUBCONTRACTOR shall follow SAE and/or
Client's requirements and obtain SAE approval before commence the work.

6.3 The SUBCONTRACTOR shall maintain the temporary facilities at his own cost and expenses.

6.4 Subcontractor shall demolish the temporary facilities after the work completed or
requested by SAE

6.5 Temporary facilities demolition, material return shipping, unused material disposal, area
cleaning and area reconditioning shall be Subcontractor's responsibility and borne at
SUBCONTRACTOR's own cost and expenses.

7. SCHEDULE

- SUBCONTRACTOR shall perform and complete the Work in accordance with Attachment 1
Project Master Schedule.
- Based on the Project Master Schedule and construction milestone required, the
SUBCONTRACTOR shall prepare and submit a Construction Detail Schedule for review and
approval.
- If in CONTRACTOR's evaluation and or audit the SUBCONTRACTOR's work performance does
not meet the schedule, the SUBCONTRACTOR shall make necessary efforts to recover the
progress schedule in SUBCONTRACTORs cost.

8. WORK EXECUTION

8.1. The SUBCONTRACTOR shall carry out and complete the Work in a professional manner that
ensures quality, safety and schedule. Regarding the quality case the SUBCONTRACTOR shall
pay attention the QA/QC requirement and Safety Environmental Procedure.
For this purpose, upon the requested by SAE, the SUBCONTRACTOR shall submit for approval
execution procedures for various work items. The procedures are prepared on the basis of
the given specifications and drawings before the execution of those items of work

8.2. Interface with other


During the execution of the work, CONTRACTOR and other SUBCONTRACTORs will be
working simultaneously at the job site. In such event the SUBCONTRACTOR shall keep a
harmonious relationship with other and shall comply with CONTRACTORs instructions or
arrangements that will be given to the SUBCONTRACTOR to minimize any delay of the work
as well as harmonious relationship with community. SUBCONTRACTOR shall not claim any
additional cost that incurs as a result of such instructions or arrangements.

9. SITE INSPECTION AND CONDITIONS

9.1. SUBCONTRACTOR shall make whatever arrangements that are necessary to become familiar
with all existing and anticipated conditions and matters that might in any way affect the cost
or the performance of the work.
9.2. Any failure to duly investigate the job site, existing facilities or the foregoing conditions shall
not relieve the SUBCONTRACTOR from his responsibility of successfully performing the work.

10. DOCUMENT TO BE SUBMITTED

Subcontractor shall submit quotation with breakdown price and list of manpower and equipment
