WE, IGZABEL D. KAMID, of legal age, Filipino, married and a resident ofPurok Masagana, Brgy. Kalawag II, Isulan, Sultan Kudarat and NORIA M. TAYA, of legal age, Filipino, married and a resident of Purok Masagana, Brgy. Kalawag II, Isulan, Sultan Kudarat, after having been duly sworn to in accordance with law do hereby depose and say;
1. That we personally know the spouses BASIR KULI GUDAL and SARAH M. MAMASALAGAT, who were legally married under the laws of the Republic of the Philippines;
2. That MAHERA M. GUDAL, who was born on June 24, 2004 at Maguindanao Ave., P-3T, Bicutan, Taguig, MM; MARDIAH M. GUDAL, who was born on October 27, 2004 and MAHAH M. GUDAL, who was born on May 12, 1999 at 07 Maguindanao Avenue, P-3, Lower Bicutan, MM, are the legitimate children of the above said spouses;
3. That the person appeared as BASER M. GUIDAL; BASIR MADALIDAY
GUDAY; and BASER KALUSAMA GUDAL in the Certificates of Live Birth of his children; ; BASER KALUSAMA GUDAL, in the Certificates of Live Birth of his children and BASIR KULI GUDAL, in the Certificate of Death with Registry No: 2005-430, refers to one and the same person;
4. That the true and correct name is BASIR KULI GUDAL;
5. That we know of the foregoing facts because we are long time friends of the above- said person;
6. That we are not related to the above-said person either by affinity or by
consanguinity;
That we execute this affidavit to attest the truth of the foregoing statements and for whatever purpose this affidavit may serve best.

3. That the person appeared as ARMIA MAMALANGKAY ESMAEL, in the
Certificate of Marriage, was inadvertently and erroneously stated, and ARMIA MAMALAGKAY ESMAEL, appeared in the Certificate of Live Birth of DATUMELS, refers to one and the same person;
