Lexter Joseph G. Fabre executed an affidavit to attest to the truth of the following facts: His driver's license was confiscated in 2011 for driving an unregistered motorcycle without a license. He was issued a ticket by the LTO. The motorcycle is no longer in working condition and he did not register it anymore. The OR/CR of the motorcycle was also lost. He is executing this affidavit in good faith to redeem his driver's license and for any other just relief.

I, LEXTER JOSEPH G. FABRE, of legal age, Filipino, married, and with


present residence and postal address at B31 L11 Villa Apolonia, Naic, Cavite
after being duly sworn to in accordance with law, hereby depose and state:

That sometime last 2011, my Driver’s License was confiscated by the Land
Transportation Office while I was driving a motorcycle due to the following
violations: “Unregistered MV” and “Unlicensed Driver”;

That after the incident, I was issued a ticket (TOP) by the LTO as evidenced
by the copy attached herewith;

That due to the lapse of time, such motorcycle has not been in good working
condition and that I was not able to use it anymore, as evidenced by the
photographs of the motorcycle;

That I did not register it anymore as it was totally defective and not in
running condition;

That the OR/CR of that motorcycle was lost also;

That I am executing this affidavit in good faith and without any intention to
defraud or deceive;

As such, I am executing this Affidavit to attest to the truth of the foregoing


specifically to redeem the Driver’s License and for other reliefs as may be
just and equitable.

of January 2018 at Tagaytay City, Philippines.
