Reminders
Students applying for a Study Permit to attend a program that is longer than 6 months can elect
to undergo a medical examination prior to submitting their application. You must contact a panel
physician from the list available on the IRCC website. Once your medical exam has been
completed, the doctor will give you a document confirming that you underwent a medical exam.
You must include a copy of that document with your application.
Medical results are valid for one year; therefore, do not undergo a new medical examination if
you have taken one with an IRCC panel physician in the preceding twelve months. Also, please
be informed that undergoing an up-front medical examination does not guarantee
approval of your application, and that all costs for the medical examination are payable
by the applicant and non-refundable. You can find more information on the IRCC website,

ADDITIONAL INSTRUCTIONS TO IMM 5483 √
(Document Checklist for a Study Permit)
Document Checklist for a Study Permit (IMM 5483), duly completed and to be submitted with
your application.
As per the Document Checklist (IMM 5483), the following IRCC forms must be completed in English
or French, signed and dated:
• Application for Study Permit made Outside of Canada (IMM 1294): it must be
completed on a computer, validated to generate a barcode and printed in black and white
using a laser printer and high quality paper. Failure to include this page of barcodes
will delay the processing of your application.
You must also provide an email address at which we can contact you. Failure to provide
an email address will result in delay and possible refusal of your application.
• Family Information Form (IMM 5645): it must be fully completed with all requested
details (full name, relationship, date and country of birth, marital status, present occupation
and present full address, whether the person will accompany you to Canada or not).
Failure to comply with this directive could delay the processing of your
application.
• Schedule 1 – Application for Temporary Resident Visa (IMM 5257 – Schedule 1), if
applicable.
Military record and Form IMM 5579 - Colombian Citizens: If you had or currently have a
military rank (Air Force, Navy, Army or Police), you must attach an official résumé issued by the
Armed Forces (Hoja de Vida Oficial de las Fuerzas Armadas).
You must also provide the IMM 5579 form, fully completed in English or French, signed and
dated. The form is available at the Visa Application Centre or by e-mailing the Embassy at:
Fee payment in an acceptable format. Include the application processing fee and, if applicable,
the biometric fee.

Your valid passport accompanied by a photocopy of the information page, which includes
the passport number, the issuance and expiry dates, your photo and your biographic
data. Your valid passport must contain at least one (1) blank page. The validity of a visa cannot go
beyond the validity of the passport.
Note: Remember to keep two (2) photocopies of the information page of your passport in case a
medical exam is required.

Note: If you are required to provide biometrics information (fingerprints and digital photo), you do
not have to provide paper pictures with your application. Consult IRCC’s website on biometrics
requirements for more information.
A copy of the Letter of Acceptance from the institution or university in Canada, which must
include the program of studies, the start and end dates of studies and the tuition and related costs
for the duration of your studies.

• Proof of your legal status in Colombia or Ecuador, if you are not currently residing in
the country of citizenship of the passport submitted along with your application.
• A Study Plan written by you, indicating why you wish to go to Canada to study at the
school and in the program for which you have been accepted, your overall educational
goals, and how this Canadian program will increase your employment chances.

Solicitud de Permiso de Estudio – Puntos importantes para
recordar
Antes de presentar su solicitud para un Permiso de Estudio, por favor lea los siguientes
documentos de IRCC que se encuentran disponibles gratuitamente en nuestra página
web:
Los estudiantes que estén solicitando un permiso de estudio para asistir a un programa con una
duración mayor a 6 meses tienen la opción de realizarse los exámenes médicos antes de
presentar su solicitud. Para hacerlo, usted debe contactar a un médico de nuestra lista
autorizada que se encuentra en la página web de IRCC. Una vez haya completado su examen
médico, el médico le dará un documento confirmando que usted se realizó los exámenes. Usted
debe incluir una copia de dicho documento en la solicitud.

No es obligatorio realizarse los exámenes médicos antes de presentar su solicitud. Sin embargo,
realizarlos puede resultar en un procesamiento más rápido. Si usted no se realiza el examen
antes de presentar la solicitud, recibirá instrucciones vía correo electrónico durante el
procesamiento de la misma, si los requiere. Por favor asegúrese de incluir un correo
electrónico en el formulario de solicitud para agilizar este proceso.
Los resultados médicos son válidos por un año. Por lo tanto, no se realice los exámenes médicos,
si ya los había completado con uno de los médicos autorizados por IRCC en los últimos doce
meses. Además, tenga presente que la realización de exámenes médicos previos a su
solicitud no garantiza la aprobación de la misma y que todos los costos del examen
médico son pagados por el solicitante y no son reembolsables. Puede encontrar más

• Copia del Registro Civil de Nacimiento, que incluya el nombre de ambos padres, o
una copia de los documentos de custodia del menor, si aplica.

sus metas educativas generales y cómo este programa canadiense puede aumentar
sus oportunidades laborales.
