1. The Form must be prepared in three copies, computer-printed or typewritten.
2. The date must be in MM/DD/YY format.
3. The approved gate pass shall expire 7 days from the date of approval/issuance.
4. Use additional form for items that cannot be contained in the first form.
5. Tampering of the document is strictly prohibited. Each correction must be countersigned.
