    Sir,

    This is to report the incident which occurred at around 3:30pm of the above date. Our
    hydraulic excavator (VOLVO EC55-B) while performing excavation at site unfortunately the
    engine was shut down. The operator Mr. Armando Obemio noticed fire came from engine
    starter and took the equipment fire extinguisher inside the cabin and immediately stop the fire
    and report the incident to the undersigned. Upon my investigation,some electrical wiring,
    ignition switch and 1 unit engine starter was burned. We suspected that electrical problem and
    extremely heat was the primary cause of the occurrence of fire.
