Major research directions:

1. Epidemiology of substance abuse in India

Contribution - Documenting the hazardous patterns of alcohol misuse (including the contribution of illicit beverages) in India
that has resulted in alcohol being one of the five major non-communicable diseases contributing to disease burden in the
country. Findings have resulted in a greater appreciation of the public health and economic consequences of alcohol misuse
and have impacted public policy at the national level (including publication of the Alcohol Atlas of India) and been reflected in
changes in international databases (WHO; GENACIS)

2. Neurobiology and genetics of alcohol dependence.

Contribution - Definition and characterization of a common vulnerability diathesis [the “externalizing syndrome”] which
underlies substance dependence, other disruptive disorders and perhaps even bipolar mood disorder as a function of gene x
environment mediated brain developmental delay. This has involved a sustained twenty year study of multiple
endophenotypes [brain morphology (MRI volumetry) – metabolism (glutamate/GABA ratio on magnetic resonance imaging) –
function (EEG, ERP, disinhibition studied using Transcranial magnetic stimulation and fMRI), autonomic function (heart-rate-
variability) – externally manifest behaviors (clinical and family studies) and the peculiar response to alcohol and other drugs
(subjective response to ethanol)] . Influenced and trained young Indian researchers to extend this work and impacted
international research directions.

3. Translational research
3.1. Baclofen in the treatment of alcohol and other drug disorders
3.2. Atomoxetine improves treatment outcome in early onset substance users
3.3. Alpha-theta biofeedback and yoga in improving externalizing diathesis
Contribution– Study of newer pharmacotherapies and behavioral interventions to the bedside, which are gradually becoming
standard practice.

centers of the Ministry of Health & WHO.

D. Dissemination and public awareness:

1. Through presentations to professional colleagues, the bureaucracy and lay public- helped to strengthen the nascent
discipline of Addiction Psychiatry in India and influence public discourse on the public health impact of alcohol and drug
abuse.
2. Designed and run a website on the Deaddiction Centre, NIMHANS, which also is a repository for published and unpublished
research in substance abuse from India.
3. Through radio-talks, targeted interviews - maintained a prominent media profile for the cause of Addiction Psychiatry and
Tobacco cessation.
4. Created treatment provider networks in Bangalore city, and throughout the southern states for information sharing and
establishing common minimum standards.
5. Scripted and directed the film on NIMHANS for it’s golden jubilee

E. Service Delivery
1. Served as a practicing psychiatrist in the National Institute of Mental Health and Neurosciences, Bangalore, specializing in
the field of Addiction psychiatry – caseload includes patients of addiction psychiatry and general psychiatry (especially
bipolar spectrum disorders, adult attention-deficit disorders).
2. Developed, established and popularised evidence based treatment protocols, using long-term pharmacological management
and group therapies, which are now replicated by service providers throughout the country.
3. Special interventions for children, adolescents and young adults with disruptive/externalizing behaviors
4. Practiced individual psychotherapy (psychodynamic ; cognitive behavioral therapies and motivational enhancement therapy)
and group therapies for relapse prevention as well as family therapy with families of substance users.
5. Special interest in interventions with drug-abusing street children – working with non-governmental agencies delivering care
to street and working children
6. Managed tobacco cessation clinic
7. Managed the electrophysiology facility [EEG, ERP and EEG biofeedback] of the Department of Psychiatry, NIMHANS

2. Field level experience in co-ordinating post -disaster mental health care delivery – as team-leader of the NIMHANS group for
psychosocial rehabilitation (designated by the Govt. of India) in the Andaman and Nicobar islands post-tsunami.
3. Innovations- Collaborated with hardware and software professionals to build an indigenous EEG/ERP apparatus for
quantitative EEG research and a paperless digital EEG system for the psychiatry department.
4. Innovations- Collaborated with Bharat Electronics Ltd. and Picopeta computers to build a flexible data-entry system on
portable computers for mental and behavioral disorders.
