    GENERAL INSTRUCTIONS

    I. The candidate should send Form 1 for Acceptance (or) Form 2 for Decline by
    post. Other forms may be furnished at the time of Registration.

    II. Reporting for Admission/ Registration:

    The candidate is required to report for Admission / Registration at 24.7.2019


    (Wednesday) at 9.00AM at IIT Hyderabad, Kandi, Sangareddy, Telangana-502285

    III. Certificates:
    The candidate is required to bring all the following original certificates / documents
    along with a set of Photostat copies for verification at the time of Registration. The
    originals will be verified and returned.

    (a) Check List Form


    (b) Registration Form
    (c) Fee Payment receipt.
    (d) Offer of admission letter
    (e) Anti-Ragging Form
    (f) Non – using of Power Vehicle Form
    (g) Medical Fitness Certificate
    (h) Declaration by Parent (Form 03)
    (i) SSLC / SSC / Matriculation certificate.
    (j) Intermediate / 10+2
    (k) Degree Certificate.
    (l) Post-Graduation Certificate : Pass Certificate / Provisional / Consolidated / Semester

    (m) Qualifying Examination Proof : GATE/CSIR/UGC/Inspire /others
    (n) Course Completion Certificate: (Form 04)

    (Form 04 - All the candidates admitted in Ph.D program at IIT Hyderabad should have
    completed all the examinations including Viva-voce by July 2019. If the results of the
    final examination are not yet announced, a Course Completion Certificate from the Head
    of your Institution must be produced. Such candidates must submit the Provisional
    Certificate on or before August 2019) otherwise the candidate is not eligible for
    admission even if the offer of admission to Ph.D is issued by IIT Hyderabad.

    (o) If the candidate is employed and joining PhD program as sponsored / self-sponsored
    must submit NOC and Experience Certificate from present employer
    (p) If the candidate is employed and joining PhD program as full time regular candidate,
    he/she must submit Experience Certificate /Relief Certificate from the employer
