JOINT AFFIDAVIT
We, ZENAIDA B. REVILLA, of legal age, married and a resident of Brgy.
Macayug, Mangaldan, Pangasinan and ROLANDO M. ESCAÑO, also of legal age, married, Filipino citizen and a resident of Brgy. Macayug, Mangaldan, Pangasinan, under oath do hereby depose and state:
1. That we are residing in Brgy. Macayug, Mangaldan, Pangasinan,
since birth (for the first affiant/and more than 25 years (for the second affiant);
2. That we personally know CECILIA QUINTO BILLOSILLO, the fourth
child of the deceased Spouses Ricardo E. Billosillo and Severa Quinto;
3. That all along, the said child, has been using her known name CECILIA QUINTO BILLOSILLO from the elementary grades, high school and in important documents that she signed;
4. That recently, when the said child secured her Certificate of Birth from the Philippine Statistics Authority, her name appearing therein is CECILIA VILLOSILLO and that her father’s name is RICARDO VILLOSILLO;
5. That we say that there is a typographical error in writing the
surname of said Cecilia Villosollo and her father, Ricardo Villosillo in that the correct surname of said persons are BILLOSILLO;
6. That at present, the said Cecilia Quinto Villosillo is married to
Alfredo M. Magno for which reason, her married name is CECILIA BILLOSILLO- MAGNO;
7. That we are executing this Joint Affidavit to establish the fact that the names CECILIA QUINTO VILLOSILLO, CECILIA QUINTO BILLOSILLO, and CECILIA BILLOSILLO-MAGNO, refer to one and the same person, and for whatever legal purpose that this Joint Affidavit may serve.
