    Step 1
    Submit the following admission requirements:
     Form 137 for Recent High School Graduates (original & photocopy)
     Transcript of Records - Professionals & College Level Students (original & photocopy)
     Certificate of Good Moral Character
     2 pcs. Recent 2 x 2 ID Picture
     NSO Certificate of Live Birth or Marriage Contract (original & photocopy)
     Laboratory Results
    - Drug Test
    - Hepa A, Hepa B Results (HBsAG, ANTI-HAV Igm)
    - Chest X-ray
    - CBC, Urinalysis, Stool
    Step 2
     Submit all requirements to ICA Administrative Office
     Fill up the Application for Admission Form
     Undergo the admission interview with the ICA Director or designated officer
     Submit Laboratory results to the University Clinic.
     Secure admission card from ICA office
     Proceed to Business Office for payment
    Step 3
     Take the Psychological Test (c/o Guidance Office) – P 300.00
