Petitioner FLORANTE FALLER SISON, by counsel, to this
Honorable Court, most respectfully states:
1. In relation to the present petition for declaration of nullity of

bigamous marriage, petitioner most respectfully requests for
the issuance of a subpoena duces tecum directing and/or
requiring the:
a. Legal Services Division and/or Office of the Civil Registrar

General of the Philippine Statistics Authority, with office
address at PSA-TAM Bldg., East Avenue, Quezon City, 1101;
and/or
b. Philippine Statistics Authority National Capital Region Field

Office located at Philippine Bible Society (PBS) Building,
Ramon Magsaysay Boulevard, Sta. Mesa, Manila 1016,

the particulars of whom are as follows:

Spouse: ALICIA ARCAN VELASCO

2. Said document is being required to prove that said Bryan Dy

Asis, husband of herein respondent Alicia Arcangel Velasco,
was very much alive at the time the respondent contracted her
bigamous marriage to the petitioner on April 12, 1983.
PRAYER
WHEREFORE, premises considered, it is most respectfully

prayed that this Honorable Court issue a subpoena duces tecum
directing the Office of the Civil Registrar General and/or the Legal
Services Division of the Philippine Statistics Authority to submit a
Certification and/or Advisory of whether or not it has a record of
death of one Bryan Dy Asis.
Other reliefs, just and equitable under the circumstances are

likewise prayed for.
