COVID-19 EMERGENCY ASSISTANCE FOR CURRENTLY ENROLLED STUDENTS

The COVID-19 Emergency Assistance fund helps currently enrolled students with extraordinary one-time education-related costs that have arisen as a result of
the COVID-19 pandemic. At this time the maximum amount of emergency assistance that will be provided by this grant is $3,000.

Note for undergraduate students: If your family’s financial circumstances have changed due to a change in income, please submit an Appeal Concerning Expected
Contribution through FAST.

Describe how COVID-19 has created unexpected costs associated with your enrollment at USC. Attach a separate document if more space is needed.

I live at Cardinal Gardens apartments off-campus housing and had left campus to go home for spring break, so I was not allowed to
return to the apartment. I have been subleasing a spot for now (lease agreement attached, I am renting Rex Wang's room), and I need
financial support because I am also paying for rent for the apartment and am not able to get to campus to access the dining halls
without risking exposure, as I don't have any form of transportation.

Log in to your Financial Aid Summary and Tasks (FAST) page at financialaid.usc.edu and follow the instructions in the Document Library to upload this form,
including attachments. The Financial Aid Office will review your request and may contact you for further information. If funding is approved, the grant funds
will be disbursed to your student account. If you have not already signed up for eRefund (Direct Deposit) on USCe.pay, please do so in order to receive your
funds immediately. Disbursements from this fund will not be applied to an outstanding balance on your student account.
