ANESTESI PADA LANSIA
Abstrak
Para lansia mengalami peningkatan bedah yang komplek dengan lebih banyak kematian dan morbiditas
dibandingkan dengan populasi dewasa muda. Pada artikel ini, kita diskusikan perubahan fisiologi yang
terjadi di pasien lansia, dan bagaimana hal ini mempengaruhi teknis anestesi. Resiko perioperative pada
lansia didiskusikan dengan focus pada bedah darurat dan kelemahan.

Demografi dari populasi Inggris berubah, menghadapi tambahan tekanan pada sumber NHS
(National Health Service – Pelayanan Kesehatan Nasional) yang terbatas : 1.6 juta penduduk sekarang
berusia diatas 85 tahun, dibandingkan pada hanya 200.000 saat NHS didirikan pada tahun 1948. Jumlah
lansia yang menjalani operasi bertambah dan pada 2014-2015, lebih dari 75an tercatat dari seluruh prosedur
bedah dilakukan di Inggris. Lansia menjalani penambahan prosedur bedah yang kompleks, dan bedah pada
lansia membawa resiko yang lebih besar pada kematian dan morbiditas daripada populasi dewasa yang lebih
muda.
Presentasi pada bedah darurat menjadi semakin umum pada pasien lansia, dengan resiko
perioperative terlalu tidak proporsional pada grup pasien ini. Trauma besar (didefinisikan sebagai besarnya
trauma dengan skor lebih dari 15) sekarang menjadi semakin umum terjadi pada umur 65an daripada pasien
yang lebih muda, dengan jatuh dari ketinggian kurang dari 2m mekanisme yang paling umum. Resiko
perioperative sangat tinggi pada prosedur kolorektal darurat, dengan tingkat kematian lebih dari 40% pada
octogenarian.
Desain dari alveolae juga berubah seiring usia. Jumlah permukaan alveolae berkurang dan terdapat
penebalan pada membran alveolar-capillary yang halus, dimana keduanya mengurangi kapasitas pembauran
paru-paru dan berkontribusi pada peningkatan A-a gradien.
Hasil dari perubahan-perubahan ini adalah gambaran klinikal lebih seperti penyakit paru obstruktif
kronis ringan (mild chronic obstructive pulmonary disease (COPD)), yaitu sebuah peningkatan sisa volume
dan fungsional kapasitas sisa (FRC). FRC adalah titik dimana kekuatan elastic bagian dalam paru-paru
sesuai dengan kekuatan bagian luar yang dihasilkan oleh tulang rusuk. Pada lansia, FRC terjadi pada volume
paru-paru tertinggi: oleh karena itu diameter depan-belakang tulang rusuk bertambah, dengan perataan dari
hemidiafragma. Kyphosis dapat pula berkontribusi untuk merugikan mekanisme paru-paru. Serupa dengan
pasien dengan COPD, perubahan mekanikal ini membuat pernapasan ini menjadi kurang efektif, dengan
sebuah penambahan pada energi yang digunakan selama pernapasan. Perubahan ini, bersamaan dengan
antar tulang iga dan atrofi otot diafragma, membuat lansia lebih rentan terhadap kelelahan pernapasan.
Kardiovaskular
Fisiologi : Arteri menjadi kurang elastis seiring usia sebagai tunica media mengalami fibrosis. Hal
ini mengarah ke sebuah peningkatan pada tekanan darah sistolik. Tekanan darah tinggi sistolik memiliki 2
konsekuensi utama:
Ventrikel kiri seharusnya menghasilkan tekanan lebih besar untuk mengeluarkan darah kapan
aorta yang mengeras.

yang tercermin menjangkau jantung pada diastole awal, dan merupakan hal yang

ventrikel kiri yang harus memompa.
Pada respon kenaikan afterload, ventrikel kiri mengalami hipertropi. Ventrikular hipertropi merusak
relaksi diastolic dan ventrikel kiri menjadi sangat tergantung pada kontraksi atrial untuk pengisian.
Gangguan fungsi fisiologi diastolik dapat diperburuk oleh penyakit komorbiditas, sebagai contoh hipertensi
sistemik dan kardiomiopati iskemik.
Terdapat penurunan kemampuan untuk meningkatkan curah jantung dalam respon terhadap
hypovolemia, disebabkan oleh :

oleh fibrosis dari simpul sinoatrial dan pengurangan yang besar pada jumlah dari sel pacu jantung.
Timbulnya fibrilasi atrium dapat sangat merusak pemenuhan dari ventrikel kiri, dengan konsekuensi
pengurangan pada curah jantung.
Pertimbangan obat bius:

menggunakan perantara induksi intravena, namun pada teori meningkatkan kecepatan dari
permulaan induksi gas. Pada kenyataannya, namun, durasi dari inhalasi induksi biasanya
berkepanjangan disebabkan karena pengurangan difusi alveolar dan peningkatan
ketidakcocokan V/Q yang terjadi pada usia lanjut.
Lansia lebih rentan terhadap hipotensi dari periode prosedur bedah (intraoperative), terutama
pada induksi dari bius, dan kurang mampu memberikan kompensasi yang memadai.
Kekurangan responsi jantung ke beta-agonis berarti bahwa obat seperi ephedrine (campuan
alpa- dan beta-agonis) menjadi kurang efektif seiring usia lanjut.
Penggunaan dari pemantauan keluaran jantung esofagus doppler menjadi kurang akurat pada
lansia, sebagaimana aorta kurang dapat dikendalikan.
Kombinasi antara perubahan dari kardiovaskular dan fisiologi pernapasan pada hasil penuaan
dalam kekurangan serapan oksigen dan pengiriman oksigen, dan dengan demikian resiko
perioperative dari miokardial dan iskemik otak.
Neurologis
Fisiologi : Terdapat penurunan progresif pada massa otak sekitar 50 tahun. Pada lansia, hal ini
menampilkan atrophia otak pada CT. Meskipun terdapat adanya penurunan dalam zat abu-abu dan putih,
yang mengakibatkan penurunan kognitif biasanya secara sederhana berpengaruh pada memori jangka
pendek. Aliran darah otak (CFB) berkurang 10-20%, sebagaimana CFB tergabung erat pada tingkat
metabolisme otak, sebagai sebuah hasil dari penurunan pada massa otak. Sintesis pada beberapa
neurotransmiter (misalnya dopamin) berkurang, sebagaimana jumlah reseptor dan tempat pengikat. Hasil
kekurangan syaraf dopaminergik pada penyakit Parkinson, sementara kekurangan kolinergik neuron terlibat
dalam perkembangan dari penyakit Alzheimer. Sesungguhnya, pelemahan kognitif umumnya menjadi
meningkat seiring usia, mempengaruhi 20% dari pasien yang berusia lebih dari 80 tahun. Kelemahan panca
indra juga umum : ketulian sangat umu pada lansia dan kelemahan penglihatan mempengaruhi sekitar 50
dari pasien yang berumur lebih dari 75 tahun.
Lansia lebih mungkin mengalami disfungsi neurologis setelah tindakan bedah :
Igauan didefinisikan sebagai gangguan dari kesadaran yang disertai dengan perubahan
kognisi yang tidak bisa dipertanggungjawabkan untuk oleh sebuah dementia yang sudah ada
sebelumnya atau yang berkembang. Igauan setelah operasi (POD) terjadi pada lebih dari 20%
dari lansia usia diatas 65, biasanya berkembang di beberapa hari pertama setelah operasi, dan
biasanya hanya sementara. Biasanya memiliki kejadian fluktuatif, dengan ritme sirkadian
yang abnormal, kurang perhatian, disorientasi dan lemah ingata. POD mungkin di sub-
klasifikasi sebagai hiperaktif, hipoaktif (dimana membawa tingkat kematian yang tinggi) atau
campuran kedua variasi. POD secara independen di asosiasikan dengan peningkatan
mortalitas, peningkatan lamanya tinggal di rumah sakit, disabilitas fungsional dan
melaksanakan perawatan dalam jangka panjang. Faktor resiko untuk pengembangan dari
POD termasuk jenis dari beda (kelompok resiko tinggi seperti operasi jantung, operasi gawat
darurat dan fraktur pinggul lanjutan) dan faktor pasien secara spesifik : bertambahnya usia,
disfungsi kognitif yang sudah ada sebelumnya, pencapaian pendidikan yang rendah, stroke
sebelumnya.
Penurunan kognitif setelah operasi (POCD) adalah sebuah gangguan memori yang halus,
konsentrasi dan pengolahan informasi biasanya terjadi hingga beberapa minggu atau
beberapa bulan, hal tersebut berbeda dari igauan dan demensia. Kejadian dari POCD sulit
untuk ditentukan, karena sebagian tidak ada kriteria formal untuk penilaian dan diagnosa,
namun dipertimbangkan ke dalam urutan 10% saat 3 bulan. Pengenalan dari PODC sangat
penting sebagaimana hal tersebut diasosiasikan dengan sebuah peningkatan mortalitas dan
dapat mengakibatkan turunnya fungsional pada kondisi sosial, sebagai contoh dari perawatan
di rumah sampai dengan perawatan perawat. Penyebab POCD tidak diketahui dan beberapa
hipotesis ada. Faktor resiko dari pengembangan pada POCD sama dengan POD, dengan
bedah jantung membawa resiko lebih besar. Sementara mencoba untuk memikirkan untuk
menghindari anestesi pada umum mungkin dapat mencegah POCD, penelitian gagal untuk
menunjukkan bahwa anestesi tulang belakang memiliki efek proteksi ketika dibandingkan
dengan anestesi umum.
Ginjal
Fisiologi : Dari umur 30, terdapat kehilangan progresif dari glomeruli dengan reduksi yang sesuai
pada aliran darah ginjal. Hasil pada berkurangnya kecepatan filtrasi glomerular dan pembersihan kreatinin,
namun konsentrasi serum kreatinin masih sama tertinggal dengan jumlah kira-kira hampir sama dikarenakan
oleh pengurangan bersamaan pada massa otot. Sebagai tambahan, ginjal khususnya dapat menjadi rentan
oleh penyakit komorbiditas (seperti hipertensi dan diabetes), dan dengan efek nefrotoksik dari manajemen
farmakologis pada komorbiditas (seperti diuretik, penghambat ACE, NSAIDs). Nefropati obstruktif sangat
umum pada pria lanjut usia, dikarenakan hipertropi protat jinak.
Managemen cairan lebih menantang pada lansia:
Responsif ginjal diubah ke zat vasoaktif : efek dari stimulus vasokonstriktor ditingkatkan dan
zat vasodilator dikurangi.
Penanganan sodium terganggu, dan ginjal menjadi kurang responsif terhadap ADH dan
aldosteron.
Hematologis
Fisiologi : Anemia adalah hematologi abnormalitas yang sangat umum pada lansia, dengan
prevalensi sebesar 20.1% pada pria lanjut usia dan 13.7% pada wanita lanjut usia. Kekurangan konsentrai
hemoglobin mungkin terkait kepada penurunan fisiologis pada hematopoiesis sumsum atau hasil dari diet
yang buruk, namun anemia seharusnya tidak dianggap sebagai bagian normal dari penuaan. Anemia pra
operasi harus di investigasi untuk meniadakan pendarahan pencernaan, untuk contoh sebagai hasil dari polip
kolon atau karsinoma, karsinoma lambung, angiodysplasia atau penggunaan NSAID.
Pertimbangan ahli anestesi.
Anemia di asosiasikan dengan perioperatif infark miokard, jatuh, dan gangguan
penyembuhan luka dan rehabilitasi yang buruk.
Penggantian zat besi sebelum operasi harus ditawarkan pada pasien anemia: penggantian
secara oral lebih baik, namun jika tidak cukup waktu sebelum bedah darurat maka intravena
zat besi dapat dipertimbangkan.
Pedoman dewasa negara Inggris merekomendasikan transfusi saat ambang hemoglobin pada
70 g/L.
Hati
Fisiologi : Terdapat adanya pengurangan pada massa hati dan aliran darah hati pada usia lanjut.
Hasil pengurangan pada sintesis plasma protein hati ini, dan metabolisme hati dari beberapa obat.
Pertimbangan ahli anestesi :
Pembersihan hati dari banyak obat anestesi berkurang. Sebagai contoh, zat pemblokir
aminosteroid neuromuskuler mungkin dapat beraksi dengan durasi lebih lama, memandatkan
secara rutin monitoring neuromuskuler. Sebaliknya, durasi dari aksi (cis-)atracurium tidak
berpengaruh dengan usia karena independen pada metabolisme hati.
Penurunan konsentrasi albumin mengubah distribusi dari banyak obat yang terikat pada
protein.
Penuruan konsentrasi plasma cholinesterase mengakibatkan lambatnya metabolisme dari
mivacurium dan suxamethonium, meskipun hal ini langka secara signifikansi klinis.
Termoregulasi
Fisiologi : Termoregulasi terganggu seiring bertambahnya usia. Adanya pengurangan pada produksi
panas, sebagai tingkat metabolisme basal berkurang sekitar 1% tiap tahun. Hal ini diperparah dengan
peningkatan pada kehilangan panas, yang disebabkan oleh ambang vasokonstriksi periferal dan kekurangan
responsif dari vasokonstriksi periferal.
Pertimbangan ahli anestesi:
Orang yang lebih tua lebih beresiko pada hipotermia intraoperatif dan pemanasan aktif
intraoperatif harus dipertimbangkan, bahkan dalam prosedur yang singkat.
Pada periode pasca operasi, lansia kemungkinan mengalami kekurangan massa otot untuk
menggigil secara efektif atau mereka mungkin memiliki kekurangan kardiopulmoner
cadangan untuk memenuhi peningkatan kebutuhan oksigen untuk menggigil.
Farmakologi
Perubahan terkait usia dideskripsikan dampak di atas pada farmakokinetik dari banyak obat. Lansia
memiliki penurunan dalam air tubuh total dengan pengurangan volume dari ruang tengah : bolus yang
diberikan dari obat yang larut dalam air akan menghasilkan konsentrat serum obat yang lebih tinggi. Lansia
juga memiliki proporsi lemak tubuh yang tinggi, yang meningkatkan volume distribusi dari banyak obat
yang larut dalam lipid, dengan demikian memperpanjang aksinya. Sebagai tambahan, distribusi dari obat
yang sangat terikat pada protein berubah pada konsentrasi peredaran albumin (pengikat utama protein untuk
obat yang bersifat asam) berkurang seiring usia, ketika α-1-acid glikoprotein konsentrat (pengikat utama
protein untuk obat dasar) bertambah.
Pertimbangan ahli anestesi : Perubahan pada farmakokinetik pada lansia memilik dampak untuk
anestesi :
Bolus yang diberikan untuk hasil zat induksi intravena pada konsentrasi plasma tertinggi,
yang menyebabkan hipotensi. Oleh karena itu dosis untuk zat induksi dikurangi.
Efek dari benzodiazepin yang diberikan saat induksi dari anestesi dapat bertahan pada masa
setelah operasi.
