    KNOW ALL MEN BY THESE PRESENTS:

    hereby NAME, CONSTITUTE, and APPOINT ___________________, Filipino, of legal
    age, with address at ______________, for me and in my stead, to do and execute all or
    any of the following acts, deeds and things, to wit:

    1. To represent me in the civil and/or criminal case that I will be filing against
    ___________________ before the appropriate court in the Philippines;

    2. To sign and/or verify the complaint-affidavit, petition, motion or any


    appropriate pleading in the afore-mentioned civil and/or criminal case related
    thereto that may be filed before the trial court, Court of Appeals or the
    Supreme Court;

    3. Appear and represent me in the mediation proceedings, preliminary/pre-trial


    conference, judicial dispute resolution and in the hearings and every stage of

    4. To negotiate for possible amicable settlement of the case under such terms
    and conditions mutually acceptable to the parties, and to execute and sign
    the necessary compromise agreement, if any;

    documentary exhibits;

    7. To consider the propriety and necessity of availing the various modes of


    discovery procedure;

    To do any and all acts as would help expedite the resolution of the case.
