$25 Biweekly
AVAILABLE TO FULL-TIME ASSOCIATES AND THEIR SPOUSES
WHO ARE ENROLLED IN A HOME DEPOT MEDICAL PLAN.1

STEP 1 TAKE THE ORANGELIFE QUESTIONNAIRE

• The link to the Questionnaire will be available after you complete


your enrollment session online.
•You must complete within 15 days of your enrollment.

STEP 2 PARTICIPATE IN THE HIDDEN HEALTH RISK SCREENING

• Participate in a Screening at:


— Home Depot locations hosting an onsite event in Jan.
— A Quest Diagnostics Patient Service Center that offers
biometric screenings
— Your doctor’s office
—please note you will have to bring your Doctor Screening Kit with you
You will need to complete Step 2 by Feb. 3, 2017 to continue receiving your credit.

STEP 3 TAKE ACTION BY COMPLETING YOUR HEALTHY LIVING CHECKLIST

1 Bronze Medical plan enrollees are not eligible for the Healthy Living Credit.

If you do not complete your OrangeLife Questionnaire by Jan. 15, the steps required to earn your credit may be modified. Log on at livetheorangelife.com
and click the Healthy Living tile for your steps.

Participation in the Healthy Living Credit steps is completely voluntary. Choosing not to participate will only result in individuals not receiving the Credit.
Note: If your physician believes that participating in the Screening or completing the Checklist items is medically inappropriate in your particular sit-
uation, you can complete a Physician Waiver Form. A physician’s signature is required, and the form is available at livetheorangelife.com or by call-
ing the Benefits Choice Center at 800-555-4954.
