    I, RIC ROSARIO ABAD, a Filipino citizen of legal age, single, residing at BLK 29
    LOT 17 SORRENTO VILLAGE, BURGOS MONTALBAN, RODRIGUEZ RIZAL, after being
    sworn to in accordance with law, depose and say that:

    1. That I am the biological father of ARON ANDREI R. ABAD, never got married, in
    fact my relationship to my son mother never become successful and have never
    communicated ever since the separation;

    2. I was thereafter left alone with the sole responsibility of parenthood but with the
    guidance and help of my family and up to present, I am solely providing for the needs
    of my son;

    3. I am executing this Affidavit to attest to the foregoing facts and for the purpose of my
    son study to have a school voucher for his study at THERESIAN SCHOOL OF
    CAVITE.
