SUBJECT: GUIDELINES ON THE RELEASE OF AID TO INDIVIDUALS IN CRISIS


SITUATION (AICS) IN LINE WITH DSWD MEMORANDUM CIRCULAR.

RATIONALE:

The Local Government Units may be designated by the National Government as implementing agencies in
its National Programs pursuant to Section 17 of Republic Act No. 7160 or the Local Government Code of
1991 wherein LGUs shall exercise such other powers and discharge such other functions and
responsibilities as are necessary, appropriate, or incidental to efficiently and effectively rendered the basic
services and facilities currently vested upon them.

OBJECTIVE:

The, issues this Memorandum Order to give clarifications and guidelines as to the release of the Aid to
Individuals in Crisis Situation (AICS) specifically those released through cheques (cash) in line with
DSWD Memorandum Circulars. The LGU shall:

1. Provide guidance on who shall be named as payee in exceptional circumstance(s) in the cheque for
release as financial assistance to the target beneficiary as defined under DSWD Memorandum
Circular No. 11 Series of 2019.

DEFINITION OF TERMS:

1. Target Beneficiaries1 – are individuals and/or families who are indigent, vulnerable,
disadvantaged or are otherwise in crisis situation based on the assessment of the Social Workers.
2. Exceptional Circumstances – wherein, by a valid reason, the beneficiary cannot personally claim
or encash the financial assistance release through cheque, due to:
a. Disability (PWD);
b. Insanity/ Embicility
c. Minority (below 18 years old)

Or when the Beneficiary opted to directly release the assistance to the following concerned
institutions:

a. Hospitals
b. Funeral Parlors
c. Attending Physicians/ Doctors
d. Other analogous circumstance

3. Authorized Person of the Beneficiary – is the person with proof of authority by the beneficiary,
in his/her absence, to claim the financial assistance on his/her behalf and shall be named as payee
therein.

DOCUMENTARY REQUIREMENTS FOR EACH ASSISTANCE 2:

Type Documents
Transportation  Any valid identification card of the client / person to be interviewed: and
Assistance  Police Blotter; or
 Police Certification (for victims of pick pockets, illegal recruitment, etc); or
 Other supporting document/s such as, but not limited to, justification of the
social worker, medical certificate, death certificate, and / or court
order/subpoena.

*Barangay Certificate of Residency or Certificate of Indigency of certificate if the Client


is in Need of Assistance may be required.
Medical Assistance  Any valid identification card of the client/ person to be interviewed; and
 Case Study Report that the client is in crisis situation issued by the Department
social worker, Local Social Welfare and Development Officer or Case
Summary issued by the Medical Social Worker, for availment of services more
than ₱5,000.00.
 Medical Certificate or Medical Abstract with date of issuance, complete name,
signature and license number of the attending physician (issued within 3
months) ; and
 If payment for hospital bill – Hospital Bill/ Statement of Statement of Account
(Outstanding Balance) with complete name and signature of the Billing Clerk;
or
 If for medicines/assistive devices Prescription with date of issuance, complete
name signature and license number of the attending physician; or

 If for medical procedures – Laboratory request with date of issuance, complete
name, signature and license number of the attending physician.

*Barangay Certificate of Residency or Certificate of Indigency or Certificate of the


Client is in Need of Assistance may be required.
Burial Assistance  Any valid identification card of the client/ person to be interviewed; and
 Death Certificate (for IPs). Imam (for Moro) or Doctor or authorized medical
practitioner, in the absence of a death certificated ; and
 Funeral Contract (except for Muslim and Indigenous People performing
customary practices):

For transfer of cadaver:

 Death Certificate or Certification from the Tribal Chieftain (for IPS) Imam
(for Moro) or Doctor or authorized medical practitioner, in the absence of a
death certificate; and
 Transfer Permit (except for Moro and Indigenous People performing
customary practices), if applicable.

*Barangay Certificate of Residency or Certificate of Indigency or Certificate of the


Client is in Need of Assistance may be required.

Food Assistance  Any valid identification card of the client/ person to be interviewed; and
For residents/clients in CRCFs:
 Project proposal; and
 Food distribution list

*Barangay Certificate of Residency or Certificate of Indigency or Certificate of the


Client is in Need of Assistance may be required.
Cash Assistance for other  Any valid identification card of the client/ person to be interviewed; and
support services
Depending on the circumstance the client:
 Police Report or Bureau of Fire Protection (BFP Report/ Certification for fire
victims; or
 Passport, Travel Document (s) Certification from OWWA or the Barangay or
an proof of repatriation by the OFW ; or
 A certification from the Social Worker or Case Manager for rescued clients; or
 Police Blotter and Social Worker Certificate for Victims of On-line Sexual
Exploitation of Children.

RELEASE OF AICS FINANCIAL ASSISTANCE:

Beneficiaries who have complied with the following minimum requirements shall be entitled to the said
financial assistance which shall be released through cheque.

1. The payee shall be in the name of the beneficiary saved for exceptional circumstances, wherein
the beneficiary cannot personally claim the said cheque due to:
a. Disability (PWD);
b. Insanity/ Embicility
c. Minority (below 18 years old)

Additional requirements:
For PWD Beneficiary:
1. PWD ID duly issued by the

For Insane/Embicile Beneficiary:


1. Certification from any government phycisian on his/her insanity/embicility

Or when the Beneficiary opted to directly release the assistance to the following concern
institutions:

a. Hospitals
b. Funeral Parlors
c. Attending Physicians/ Doctors

Note that only in the following circumstances that another person shall be named as payee in behalf of
the beneficiary and subject to the provision in number 2.

2. In case where the payee falls under the exceptional circumstances and cannot personally claim the
cheque, the following shall be observed:
a. Letter Authority that indeed the beneficiary authorizes him/her to be name as payee
therein;
b. Authorized Person shall be living within the same household with the beneficiary; or a
relative of the beneficiary within the 4th degree of consanguinity who also resides in the.
c. In case of minor beneficiary, the Authorized Person shall be his/her legitimate parents, in
default of the former, his/her guardian.

EFFECTIVITY:

This Order shall take effect immediately upon signing.
