    Chennai. They Required the Sanitary / Health Certificate from your good office. Hence we
    kindly request you to issue the certificate at earliest

    1. Copy of Trade license for the year 2012-2013.

    2. Copy of food License for the year 2011-2013.

    3. Copy of Classification Project level Approval from India Tourism.

    4. Copy of Classification Application Page No: 12, Clause C.

    5. Copy of Staffs Health Report Issued by Govt Approved Doctor.
