    AFFIDAVIT OF SUPPLEMENTAL REPORT

    I, MARITES MENDIOLA, of legal age, married, Filipino Citizen and a resident


    of TAMAG, VIGAN CITY, ILOCOCS SUR, PHILIPPINES after having duly sworn in
    accordance with laws do hereby depose and state:

    That I, was born in Baguio City and whose facts of my birth was duly
    registered with the Office of the Local Civil Registrar of Baguio City under Local Civil
    Registry Code No. 7704581

    That recently, when I secured a copy of my Certificate of Live Birth from the
    Philippine Statistics Authority – Office of The Civil Registrar (PSA-OCRG), I
    discovered that my first name was BABY GIRL, it should be MARITES which is the
    truly correct one.

    That I am executing this Affidavit to attest to the truth of the foregoing in


    supplying my First Name MARITES on my Certificate of Live Birth with the PSA-
    OCRG Copy ONLY and for all legal intents and purposes this may well serve.

    IN WITNESS WHEREOF, I have hereunto set my hands this 7th day of September
    2020 in the City of Baguio, Benguet, Philippines.
