    ARIZONA  ACTORS  ACADEMY  |  Library  Usage  Agreement  
     
    ELIGIBILITY:    
    The  AZAA  library  is  offered  as  a  resource  to  students  who  are  currently  enrolled  in  
    on-­‐going  classes  and  have  an  executed  enrollment  agreement  on  file  that  
    includes  credit  card  info.    
     
    The  use  of  the  AZAA  library  is  a  PRIVILEGE,  not  an  entitlement.    
     
    USAGE  POLICIES:  
    As  per  this  agreement,  I  consent  to:  
     
    ­ Notify  Brandy  of  the  title  /  author  prior  to  borrowing  the  book  
     
    ­ Not  damage,  dog-­‐ear,  break  the  binding,  or  write  in  any  book  borrowed.  
     
    ­ Return  borrowed  book  within  two  weeks  time.  
     
    ­ Replace  borrowed  book  on  shelf  in  alphabetical  order  by  author’s  last  
    name  
     
    ­ Replace  any  borrowed  book  if  lost  or  damaged.  
     
    I  understand  that  if  I  leave  the  program  for  any  reason  and  do  not  return  any  
    outstanding  borrowed  books,  my  card  on  file  will  be  charged  $50.00.  
     
    I  agree  to  respect  and  adhere  to  the  AZAA  Library  Usage  Policies.
