    AFFIDAVIT

    We, BENITA I. RAFIÑAN, LETICIA M. ILAGAN and ENGR. EMMANUEL S. SUNGA,


    all of legal age, with address in Maliko, Mabitac, Laguna, after having been duly sworn to in
    accordance with law hereby depose and say that:

    1. Engr. Emmanuel S. Sunga is the former Irrigation


    Superintendent I of the National Irrigation Administration, Sta Maria Mayor
    River Irrigation System (NIA-SMMRIS). He likewise served as the “liaison
    officer” of the Office tasked to remit to the GSIS the contributions of all NIA-
    LRIMO together with the Master List of all paying employees.

    2. On August 18, 2006, NIA-SMMRIS requested for reconciliation of


    its record of payments for all its employees with that of the GSIS’ record. Ms.
    L. Castelo, an employee of GSIS from the Billing Department, was the one who
    conducted the said reconciliation of records.

    3. Upon securing a copy of the reconciled records from the GSIS,


    Engr. Emmanuel S. Sunga ordered the encoding in the computer of all the
    entries contained therein such as the name of the employees, the amount of
    the employees’ share and that of the Government, the Official Receipt of
    payment and the actual date of remittance of payments.

    4. The foregoing documents had corresponding original vouchers


    and other supporting papers which had been kept in the office of NIA-
    SMMRIS. Unfortunately, since the Office is located in a flood-prone area, the
    said vouchers and other supporting documents evidencing regular remittance
    of the GSIS contributions of its employees have been washed out by a series of
    typhoon and flooding that hit the area. Hence, what only remains is the file
    stored in the computer, which is also based on the records of the GSIS itself,
    hence, may be verified with the same agency.

    5. We are executing this Affidavit to attest to the truth of the


    foregoing facts and for purposes of supporting the demand of Mr. Jaime
    Realeza’s surviving heir for the adjustment of the amount of benefit due her
    and for whatever legal purpose this Affidavit may serve.

    IN WITNESS HEREOF, we have hereunto affix our signatures this 25 th day of


    February 2016, in Pangil, Laguna.
