    Bagong Bayan Mambugan Antipolo City, after having duly sworn to in accordance
    with law, hereby depose and state:

    1. That, I am the General Manager of HOLDSUN PHILIPPINES INC. a company duly


    organized and existing under and by virtue of the laws of the Philippines, with principal
    office address at #81 Gertrudes St., Bagong Bayan Mambugan Antipolo City

    2. That, We are submitting the Receipt and Update Provisional Formula of


    Conversion from Industrial Technology Development Institute once it is available;

    3. That, Due to compliance of the aforementioned office, Our Company will not
    provide it immediately yet It will take time to request it to our supplier and it will
    affect the Exportation and Liquidation of our Raw Materials to Phil-Export

    4. That this affidavit is executed to attest to the truth of the foregoing facts
