CONTRACTORS AFFIDAVIT OF NO LIABILITIES

I, REXOR STEVE B. LENUEVA, of legal age, Filipino married with residence and
postal address at P-4 La Filipina. Tagum City, Davao del Norte, having sworn in
accordance with law, depose and state:

1. That I am the owner of Blurex Construction & Supply, CONSTRUCTION OF


BATO-BATO BHS (POBLACION), SAN ISIDRO, DAVAO ORIENTAL.

2. That the implementation and efficient prosecution of all my contract work with the
said agency I hired laborers, used construction materials;

3. That I do hereby declare as of this date, I have paid all the salaries, and wagers
of all my laborer’s, have paid all construction equipment, and above all have paid
all the taxes due the local and national government in connection with all my
contract work with the Province of Davao del Norte.

4. That I finally declare that from _______________, to present, I have no


obligation and liability in whatever form or nature to anybody and to any
government office as far as all my contract works with the Province of Davao De
Oro.

5. That any loss of materials in the site is the responsibilities of the contractor.

6. That the foregoing statements are true and correct to the best of my personal
knowledge.
