INCIDENT REPORT

Case Number: R 05/04/21/0001

Incident: Robbery

At about 10:00 in the evening on 5 April 2021, I received a call from Mrs. Liezel Fuentes
regarding a Robbery happened in her house. Mrs. Fuentes said that after eating dinner
she decides to visit her daughter in coffee shop and at around 9:45 she decided to go
back in her house early to take a rest. After she parks the car in front of her house she
notice that the doorknob is destroy and discovered that her jewelry and antique figurine
was missing.

Her neighborhood Sancha Andrade at around 9:30 in the evening saw Balthazar
Armando in front of the house of Mrs. Fuentes wearing a black shirt and black backpack
bag she noticed that Balthazar Andrade’s movement is very suspicious.

Mrs. Fuentes described her jewelry as purely gold, 3 sets of necklace, earing and her
diamond ring and her 3 antique figurines. She estimated the value of his jewelry at
₱18,000,000 and the value of antique figurine at ₱5,000,000.

I conducted a survey of the crime scene and I found the screwdriver located at the side
of the cabinet where the jewelry of Mrs. Fuentes is located. I saw a broken window that
used the suspect for his leaving.

I obtained a sworn statement from Mrs. Fuentes and provided with the case number
and information.

Brief Outline of Incident

Above-named complainant, 38 yrs. old, married and resident of above-stated address,


reported an incident happened in her house that at about 10:00 in the evening on 5 April
2021, I received a call from Mrs. Liezel Fuentes regarding a Robbery happened in her
house. Mrs. Fuentes said that after eating dinner she decides to visit her daughter in
coffee shop and at around 9:45 they decided to go back in her house early to take a

VIOLATION OF RA. NO. 9165

INFORMATION

THE UNDERSIGNED accuses of the crime Violation of RA. NO. 9165 for the
possession of dangerous drug, defined and penalized under Section 5, Article ll of
Republic Act No. 9165 committed as follows:

That on or about the 14th day of April, 2021 at around 10:00 in the evening in the
Municipality of Capalonga, Province of Camarines Norte, and within the jurisdiction of
Hon. Court, the above-named accused, not being lawfully authorized by law. Did then
and there willfully, unlawfully, and feloniously have in his possession and under his
control and custody one (1) heat-sealed transparent plastic sachet containing white
crystalline substance weighting 0.30 gram, which when tested was found positive of
Methyl amphetamine (sic) Hydrochloride a dangerous drug.
