    I, MARIO MARCIAL ARGUELLES, of legal age, Filipino and a resident


    of 9024 Bankers Village, Bucandala 2, Imus City, Cavite, Philippines, after
    having been duly sworn to in accordance with law, hereby depose and say:

    1. That I am a member of the Social Security System (SSS);

    2. That I was informed by SSS that there are contributions paid by


    and attributed to UNIVERSAL ROBINA CORPORATION, which
    was never my employer;

    3. That as such, I request SSS to delete the said contributions paid


    by UNIVERSAL ROBINA, as these should go to the right and
    proper person/s;

    4. That I am executing this affidavit for all legal intents and purposes.
