    AFFIDAVIT OF REFUND OF MONEY


    I, NIHCOLE REY CASTRO SUBANG, of legal age, single, Filipino citizen and resident
    of 312 Luna St. Villanueva, Bautista, Pangasinan, after having been duly sworn to in accordance
    with law, hereby depose and say:

    1. That I am acting as representative of my mother to transact with the ELC Builders &
    Engineering Consultancy Services (ELC Builders for brevity), which is owned and operated by
    EVELYN L. CAMBAS, to build a house with a Total Contract Amount of ONE MILLION
    EIGHT HUNDRED THOUSAND PESOS (Php.1,800,000.00);

    2. That, on September 18, 2020, October 1, 2020, October 3, 2020, October 15, 2020,
    October 22, 2020 and October 30, 2020, I have paid to ELC Builders, evidenced by several
    Acknowledgment Receipts issued by the said ELC Builders as down payment for the House
    Construction which will be situated in Villanueva, Bautista, Pangasinan;

    3. That, after several months have passed, the construction of the house was stopped,
    despite paying enough money to purchase materials which will be used for the construction; and
    Up to this present time, still the said ELC Builders have not taken action and did not resume
    working for the accomplishment of the said House Construction;

    4. That, I now certain to demand a full refund of my money which I have paid to said
    ELC Builders, excluding the material which are already built and bought, within fifteen (15)
    days upon notice to ELC Builders;

    5. That I am executing this Affidavit of Refund of Money to declare solemnly the


    foregoing facts and circumstances, holding myself liable to whoever may be prejudiced for
    relying on my narrations herein.

    IN WITNESS WHEREOF, I hereunto set my hand this, 12th day of December 2020 at Alcala,
    Pangasinan.
