1. The schedule of Collegiate Periodical Examinations for First Semester of School Year 2021-2022 is as
follows:
1.1 FIRST GRADING EXAMINATIONS SEPTEMBER 14-17, 2021
1.2 MIDTERM EXAMINATIONS OCTOBER 26-29, 2021
1.3 FINAL EXAMINATIONS DECEMBER 6-7 & 9-10, 2021

1.4 LABORATORY CLASSES: Alternative or virtual laboratories were polished in the previous academic
year. Each curriculum was cautiously analyzed and with prudent judgment, the Deans scheduled and
offered courses with the laboratory component. Relative to this, the schedule of examinations shall follow
the aforementioned dates following the first hour of the class schedule.
For the final examinations, the laboratory component shall be scheduled on the 17th week of
Classes.
1.5 AUGUST 9, 2021 (Monday) all instructional materials for the AY2122A are
ready for distribution/uploading to the LMS and downloading to students once classes start.
1.6 Modules/learning packets/instructional materials MUST contain a watermark/imprint PROPERTY OF UB
per page. This will protect the intellectual property rights of the faculty members and the University.

2. Classes that do not fall under the regular schedules and whose periodical examinations cannot, as a
consequence, be held in the foregoing dates may be held at other dates within the examination week upon
prior approval of the concerned Dean. Approval must consider the availability of students for adequate time
to take the examinations.

3. Each faculty member SHOULD STRICTLY FOLLOW THE SCHEDULE OF EXAMINATIONS. Any deviations
from the foregoing schedule of examinations must have the prior written approval by the Dean concerned.
The approved schedule must be disseminated a week (at least five working days) to all concerned prior to
its implementation.

4. The office mandates holistic assessments. Summative examinations are highly encouraged to better gauge
the students’ performances. Students’ portfolios are highly encouraged to further assess the students’
outcomes vis a vis power competencies. Online teacher made examinations MUST be provided to authorized
personnel {for evaluation before online administration} as stipulated in item 8 of the Commitment to Duties

and Responsibilities for Teachers. The schedules for the submission of test questions for evaluation should
be communicated by the Program Chair to the faculty members during the start of semester’s cluster meeting.

5. Faculty members are mandated to administer their own periodical examinations. Therefore, as provided by
Memorandum C No 26 series 2017 attendance to seminars, trainings and filing of personal leaves during
examination schedules is highly discouraged.

6. Faculty members are highly requested to give a gentle reminder to students to pay their financial dues to
the University during examination schedules. The issuance of the examination permit is deferred during this
semester.

Based on Section 99, Art. XX of the Manual of Regulations for Private Higher Education (MORPHE, 2009 ed)
Re: Denial of Final Examinations; Withholding of Grades; and Refusal to Re-enroll, states that: “No higher
education institution shall deny final examinations to a student who has outstanding financial or
property obligations, including unpaid tuition and other fees corresponding the school term.”

ALL STUDENTS ARE ALLOWED TO TAKE THE FINAL EXAMINATIONS. However, to ensure uniformity in
implementing the above-mentioned guideline, and warrant that full payment of fees is strictly needed to take
the final examinations, students are required to show their proof of payment dated during the dates on
scheduled exams, except for those who chose full payments and have approved promissory documents.
Teachers need to check on the individual student’s final assessment and official receipts/deposit slips in lieu
of the FINAL EXAMINATION PERMIT. Temporary permits or alternate documents are released by the Head
of the Students Accounts’ Office.

7. Any faculty member who is not able to submit on time printed e-records/class records and grading sheets to
the respective Dean’s office shall be given sanctions as contained in Memorandum C No 21 series 2018, to
wit: “A faculty member who submits after the deadline to the respective Dean/Principal items such as
e-record/class record with grade sheets shall be charged Php 50.00 per unit per day.” The penalty starts
on the day the documents are submitted at the Dean’s office and not the date of the uploading of the grades
in the Academic Management System (AMS) portal. The deduction shall be coordinated with the payroll office
after the faculty member confirms the communication. The penalty is in addition to sanctions provided by the
Manual for Teaching Personnel (2017 edition).

a. Uploading of final numerical and non-numerical grades will be depending on the documents submitted:
1. Numerical- if the student passed or failed the course and presented a zero (0) balance final
assessment slip/clearance or presented a PROMISSORY/SAO document (with Ms. Rowena
P. Sungduan's name and signature ONLY) even with financial balance. The SAO document
is as good as a permit to take the examinations and BE ISSUED THE RESPECTIVE
GRADES.
2. Non-numerical (INC)- if the student passed the course, presented a zero (0) balance final
assessment slip or presented a PROMISSORY/SAO document (with Ms. Rowena P.
Sungduan's name and signature ONLY) even with financial balance but missed activity/ies
that need/s to be submitted to complete the requirements of the course and gain a passing
grade.
3. Non-numerical (NFE)- if the student passed/failed the course, DID NOT present any
promissory/SAO document (with Ms. Rowena P. Sungduan's name and signature ONLY),
with financial balance and with academic deficiencies. The student WILL NOT TAKE another
examination, but needs to submit proof of balance payment and needed academic
requirements (if applicable) on or before the deadline of the completion date. However, if the
student does not submit any document then s/he gets a no credit grade (ARC Manual). On
the other hand, if the student did not take the examination, then completion policy will be
applied. (Memo C no. 20 s 2014, Memo C no. 5 s 2010 and Guidelines for completion/special
examination Effective second semester SY2009-2010)
4. INC/NFE marks incurred Second semester and Short term AY2021, will be completed on
or before SEPTEMBER 10, 2021 (FRIDAY).

8. In consonance to Memo C No 09 series 2021 Re: Revision of Item 1 of Memo C No 21 s 2018 re Guidelines
in the Imposition of Penalties for Delayed Submission of Academic Reports, the deadlines for the submission
of e-records and grade sheets to the respective Dean’s office are adjusted as follows:

9. SEMESTRAL BREAK for this semester will be from December 13, 2021 to January 9, 2022. Therefore, ALL
STUDENT RELATED ACTIVITIES are PROHIBITED during the break as contained in Memorandum C No
27 series 2017 Re: Deadline for the conduct of theses defenses, submission of requirements, project
presentations and defenses, class culminating, individual or group activities & similar activities and the
teacher’s compliance of submission of all requirements at the end of the semester.

10. STRICT COMPLIANCE to the provisions of this Memorandum by all concerned is hereby enjoined.
