    AFFIDAVIT OF LOSS

    I, CECIL T. GUMAPAC, Filipino, of legal age, married and a resident of Nueva


    Vida Sur, Carmen, Bohol, Philippines, after having been duly sworn to in accordance
    with law, do hereby depose and say:

    1. That I am a bonafide holder of a Non-Professional Driver’s License duly


    issued by the Land Transportation Office (LTO), Tagbilaran City;

    2. That said Non-Professional Driver’s License was kept in my wallet for


    safekeeping but the same fell from my back pocket;

    3. That the said Non-Professional Driver’s License got lost sometime around
    July 4, 2020;

    4. That I exerted diligent efforts to find and recover my said Non-Professional


    Driver’s License, however, my efforts proved futile as until now, its
    whereabouts is still uncertain, hence, I considered it to be lost for all intents
    and purposes;

    5. That my Non-Professional Driver’s License was not confiscated by lawful


    authorities for any violation of traffic laws or rules and regulations;

    6. That I execute this affidavit voluntarily to attest to the truth of the foregoing
    statements for whatever legal purpose this may serve.

    IN WITNESS WHEREOF, I hereunto set my hand this 6 th day of July 2020, at


    the City of Tagbilaran, Bohol, Philippines.
