JAIME E. MEDINA, Filipino, of legal age, single with residence /postal address at 21 Lancelot
st., Kingsville Court, Mambugan, Antipolo City, hereinafter referred to as the VENDOR;
-AND-
RESTITUTO B. LU married to ESTELA V. LU, both Filipino, of legal age, with residence and
postal address at Tandang Sora corner Commonwealth Ave., Quezon City, hereinafter referred
to as the VENDEE;
WITNESSETH:
That the VENDOR, is the absolute owner of a portion of land, in fee simple, of a certain parcel of
land, situated in San Pablo, Castillejos, Zambales, covered by Transfer Certificate of Title No. CLOA-T-
1508 of the office of the Registry of Deeds for Iba, Zambales and declared for taxation purposes under
Tax Declaration No. 012-1767.
Whereas, the VENDOR is willing to sell to the VENDEE a portion of the aforesaid consisting of
One Thousand One Hundred Fifty Seven (1,157) square meters in the amount of EIGHT HUNDRED
THOUSAND PESOS (Php 800,000.00), Philippine Currency, which portion of property is more
particularly described as follows:

“ A parcel of land (Lot 3503-BB of the subdivision plan Psd-03-190035 being a portion of
Lot 3503 Castillejos Cadastre L. R. C. Rec. No. ) situated in the Barangay of San Pablo,
Municipality Of Castillejos, Province of Zambales, Island of Luzon.
Bounded on the NE., along line 1-2 by Lot 3503-J of the subdivision plan; on the SE.,
along line 2-3 by Road; on the W., & NW., along line 3-4-5 by Lot 3503-S (Road); on the NE.,
along line 5-6 by Lot 3503-G; along line 6-1 by Lot 3503-I all of the subdivision plan. Beginning
at a point marked “1” on plan being N. 36 deg. 15’E., 2.315.16 m. from BLLM No. 1, Castillejos
Cadastre.
Thence S. 10 deg. 12’W., 11.47 m. to point 2;
Thence S. 78 deg. 35’W., 25.01 m. to point 3;
Thence N. 15 deg. 28’E., 61.24 m. to point 4;
Thence S. 86 deg. 14’E., 16.89 m. to point 5;
Thence S. 07 deg. 09’W., 26.09 m. to point 6;
Thence S. 12 deg. 10’W., 16.15 to the pt. of beginning; containing an area of ONE
THOUSAND ONE HUNDRED FIFTY SEVEN (1,157) SQUARE METERS.
WHEREAS, the VENDOR has agreed to sell unto and in favor of the VENDEE, and the VENDEE is
willing to and /or agreeable to buy from the VENDOR, its ownership rights and interest over the above-
said property, under the terms and conditions provided hereunder;
NOW, THEREFORE, for and in consideration of the foregoing they do hereby agree, as follows:
1. That the total purchase for the above-said parcel of land shall be in the sum total of EIGHT
HUNDRED THOUSAND (Php 800,000.00.) PESOS, due and payable in the manner and within
the period as follows:
a) Php 300,000.00 option money, which shall be considered as down payment upon the
signing and execution of this Contract, the signatures hereon of the VENDOR being its
acknowledgement that she already have received from the VENDEE the said amount;
b) That upon full down the VENDOR will start the process of transferring the title in the
name of the VENDEE.
c) That the VENDEE hereby obligates himself to pay the remaining balance to the VENDOR
amounting to Php 500,000.00, in form of five (5) posts dated checks One Hundred
Thousand each check starting June 19, 2015 up to October 19, 2015 after the transfer
of title in the name of RESTITUTO B. LU married to ESTELA V. LU.
IN WITNESS WHEREOF, the parties have signed this instrument on this __ day of

ACKNOWLEDGEMENT
Both known to me and to me known to be the same persons who executed the foregoing instrument
and they acknowledged to me that the same is their free and voluntary act and deed.
This instrument, consisting of two (2) pages, including the page wherein this Acknowledgement is
written refers to a DEED OF CONDITIONAL SALE and has been signed on each and every page
thereof by the parties and their instrumental witnesses and sealed with notarial seal.
