    TERMS OF REFERENCE

    This Agreement by and between:

    The United Church of Christ in the Philippines (UCCP), as represented by Rev. Francisco
    Hernando, Executive Secretary to be referred to as “UCCP”

    AND

    ___________________, Filipino, of legal age, and with address at _________________ to be


    referred to as the “Main Moderator”

    This Agreement shall have the following provisions:

    1. Scope of Work. The work to be undertaken by the Main Moderator is to lead in the
    preparation and collection of data from the conferences and local churches which
    outlines below:

    a. Conduct the survey through a Focus Group Discussion (FGD) in conference


    councils and local churches.
    b. In close coordination with the Conference Ministers and Jurisdiction Office to
    randomly select local churches and conference councils as respondents.
    c. In selecting group respondents, he/she must consider the balance of the
    following in establishing selection criteria for FGD, especially for local churches:
    1) Small and big number of members
    2) Rural and urban
    3) Tribe, indigenous peoples, minorities, etc
    4) Local church, worshipping congregation and mission outreach.
    d. He/she must accomplish the following:
    i. Set a schedule for each local church and conference council session.
    ii. Send out the questions along with the instructions and timeframe.
    iii. Facilitate the entire process for each session. Each conference council or
    local church has the discretion on whether to conduct the FGD in virtual,
    blended or face-to-face modes. However, each respondent and
    moderator must take precautionary measures considering the COVID-19
    pandemic.
    iv. Collect the accomplished survey forms (data).
    v. Submit the data in a timely manner to Organizational Ministries at
    [email protected], cc’ing their JAMCO.

    2. Output. Consolidated answers gathered from the survey or Focus Group discussions of
    the selected respondents.

    3. Deadline of Submission. August 20-25, 2021.

    4. Fee. The contract value for the work described in the scope of work, which will be
    provided by the UCCP is ___________ PESOS (PhP ______.00).

    5. Payment. Payment will be based on the following schedule:


    a. Two Thousand Five Hundred Pesos (Php 2,500.00) upon the signing of this
    contract;
    b. Full payment within one week after submission of the Final Output or 1 st
    September 2021 whichever comes first.
