    AFFIDAVIT OF WHEREABOUTS

    I, BEVERLY V. OCADO, of legal age, Filipino and a resident of Bulala, Camalaniugan,


    Cagayan, after having been duly sworn to in accordance with law do hereby depose and say,
    THAT:

    1. I am the child of BENEDICTO N. OCADO and NAME OF MOTHER (decesased);

    2. My mother died on August 13, 2009 in Bulala, Camalaniugan, Cagayan;

    3. My parents were legally married to each other but have been separated even before my
    mother died, and my father, BENEDICTO N. OCADO, has disappeared from our lives
    since then;

    4. I only saw my father again in 2005 when I was only four (4) years old at that time and I
    haven’t seen him since then;

    5. We can no longer locate my father, BENEDICTO N. OCADO and we have no


    knowledge about his present whereabouts;

    6. I exerted diligent efforts to locate and find my father but our efforts proved futile;

    7. I execute this affidavit to attest to the veracity of the foregoing averments and for any
    purpose it may serve.
