    MAIN STEPS OF TASK POTENTIAL HAZARDS CONTROL ACTIONS NEEDED TO REDUCE RISK
    Each step is for a specific part of the task. Identify the hazards relating to each step. List the controls required to reduce the risk in each step.

    Man Basket Loss of way slip (Serious injury, Fracture). Beware to uneven surfaces. Must wear FBSH if working on heght.
    Work at Height

    Ladder, Steps and scaffolding. Ladder, steps and scaffolding inspected by the certified safety staff
    and colour code on it.
    Poor weather conditions. Do not allow work in poor weather conditions especially when wind
    speed is more than 30kph.
    Pre work Task supervisor must conduct TBT before starting the work.

    Housekeeping and other portable electrical Before starting the work, housekeeping must be carried out.
    equipment’s.
    Use of PPEs All employees must wear all required PPEs. E.g. safety helmet, eye
    protect goggles, safety shoes etc.
    Using Sub-standard self-fabricated man basket Training for attachment & use of Man basket

    Sudden Movement of man basket Only standard man basket shall be used for work height approved by the 3rd party
    (TUV)

    Adverse weather, severe storm All four lifting eyes must be attached with slings

     For each hazard, indicate who is at risk: employees (E), sub-contractor employees (SC), Members of Public (P), Others (Specify) (O)
