Check List of Documents for Lost Card Liability

Lost / stolen cards


 Duly filled and signed Claim Form by Bank
 Police Acknowledgment of any kind / First Information Report
 Transactions Summary by Bank with Date of Loss, Time & Location
 Bank System Screen shot for Blocking card highlighting date and time of block
 Dispute letter from Customer
 Investigation report of the Bank
 Bank Statement from 3 months prior to the Date of Disputed Transaction, up to the last transaction
 Bank System screen shots on the information of the number of times balance check was done (through ATM /
Internet banking / Call to Customer Care)
 Call / SMS details from the Bank to the Customer
 Action taken by the Bank:
 Charge back details to the customer (as and when the Bank has done recovery)
Fraudulent of lost/ stolen cards (In Transit)
 Duly filled and signed Claim Form by Bank
 Transactions Summary by Bank with Date of Loss, Time & Location
 Bank System Screen shot for Blocking card highlighting date and time of block
 Acknowledgment Letter from Courier / Acknowledgment copy/POD from receiver in case of wrong delivery
(Speed Post waiver) and name of the courier boy delivering the Card as well as the PIN (personal identification
number)
 Call / SMS details from the Bank to the Customer
 Bank System screen shots on the information of the number of times balance check was done (through ATM /
Internet banking / Call to Customer Care)
Counterfeit / Skimming / Phishing / Online transactions / ATM Losses
 Duly filled and signed Claim Form by Bank
 Police Acknowledgment of any kind / First Information Report (where the loss location is same as the location of
the insured person)
 Transactions Summary by Bank with Date of Loss, Time & Location
 Bank System Screen shot for Blocking card highlighting date and time of block
 Bank System screen shots on the information of the number of times balance check was done (through ATM /
Internet banking / Call to Customer Care)
 Dispute letter from Customer
 Call / SMS details from the Bank to the Customer
 Investigation report of the Bank
 Merchant details, codes from visa/master/paypal
 Bank Statement from 3 months prior to the Date of Disputed Transaction, up to the last transaction
 Passport copy (Immigration stamped pages) to check his presence in or out of country where transaction takes
place
For Online claims
 On-line complaint lodged by the customer to the Merchant website who requires authentication of the Transaction
from the customer, disputing the transaction (e.g. Pay Pal)
For ATM claims
 ATM CCTV footage (for transactions in India) – HDFC can ask for this from the customer at the time of
investigation
 Card application form with name and photograph of the insured person
For Pre-paid/Forex cards
 Call/SMS details from Bank to the customer

Note – For All type of Claims, if Any Additional Documents are required for further processing of claim then
Claims Department shall accordingly call for the additional documents from the claimant
